$15,000 Sign-On Bonus/ Full Time/

What You’ll Do

As a Home Health Physical Therapist, you will:

• Evaluate patients’ mobility, strength, balance, and functional abilities in their homes

• Develop individualized treatment plans based on each patient’s needs and personal goals

• Provide hands-on therapy to improve movement, safety and independence

• Adjust treatment plans as patients progress and their needs change

• Educate patients and caregivers on exercises, equipment, fall prevention and safe mobility

• Collaborate with physicians, nurses, therapists and other members of the care team

• Guide support staff in helping patients achieve their functional goals

• Complete accurate and timely clinical documentation

• Serve as a trusted therapy resource for patients, families, colleagues and the community

What You’ll Need

• Graduate of an American Physical Therapy Association-approved physical therapy program

• Current, unrestricted Physical Therapist license in the state of practice

• Dependable vehicle, valid driver’s license and current automobile insurance

• Ability and willingness to travel throughout the assigned coverage area

• Ability to work independently while remaining connected to a collaborative care team

• Sit, stand, bend, lift, and move intermittently, and be able to lift 50–100 lbs.
