Education 

Graduate from an accredited Licensed Vocational Nursing Program. 

Experience 

  • Three (3) years nursing experience preferred  

  • One (1) year experience with IDD Adults preferred

Position Summary:  

Breckenridge Village (BV) is a Christ-centered community serving adults with intellectual and developmental disabilities (IDD). Our LVN- PRN Team ensures that all interactions reflect Breckenridge Village’s values: Love God, Value People, Care Passionately, Work Together. 

There are times when additional medical supervision and support are needed on our Campus. The LVN-PRN provides short-term, as-needed medical coverage to help ensure the health, safety, and well-being of the individuals we serve. This position works collaboratively with BV staff and leadership to respond to medical needs, provide appropriate first aid and emergency care within the scope of LVN certification, and assist with assessing situations that may require additional medical attention. 

The LVN-PRN serves as a calm, compassionate, and professional medical resource while interacting respectfully with individuals with IDD, their families, and BV team members. The position may be scheduled for special events, periods of increased medical needs, staff shortages, or other short-term campus coverage need. 

The ideal candidate is dependable, compassionate, observant, and comfortable working with adults with IDD. The LVN- PRN must be able to respond appropriately to medical concerns, communicate effectively with the BV team, maintain appropriate documentation, and follow BV policies and procedures. 

This is a PRN, short-term position with hours based on the medical coverage needs of the Breckenridge Village campus. 

 

To successfully perform this job, the employee may be expected to perform the following, with or without reasonable accommodations in accordance with the Americans with Disabilities Act of 1990: 

 

Essential Functions: 

Perform nursing duties in accordance with nursing guidelines and best practices established by the Texas Board of Nursing for Licensed Vocational Nurses 

Perform the following duties to ensure the highest level of medical care for each resident assigned to you:  

a. Respond to injuries and illness during normal business hours  

b. Evaluate all health and medical complaints of residents and coordinate appropriate care for follow up  

c. Regarding medication for assigned residents: Manage medication ordering and delivery, ensure accurate distribution of medications, maintain adequate supplies of medication, ensure proper storage of medications, Re-label any medication that is changed with updated label, perform audits of MAR, as directed train house managers on medication administration or any other medical function necessary including any necessary training on assistance with activities  of daily living 

Maintain medical documentation for each resident assigned:  

a. Enter appointment summaries into Electronic Medical Record within 24 hours of  the appointment, including any written material or oral instruction provided  at appointment.  

b. Update medication administration records (MAR) in real time when medications  are    changed, added or discontinued 

c. Create nurse progress note, in real time, in Electronic Medical Record when any  communication regarding medical care is received from or provided to a resident,  guardian, or Breckenridge Village staff member  

d. Document health and medical complaints 

Communicate in a clear, transparent, and timely manner all information regarding each resident assigned to you:  

a. Contact the QIPD immediately when any of the following occur: Illness or injury requiring transport to Urgent Care or Emergency Department Illness that requires a medical appointment or isolation/quarantine Injury that requires medical attention or A medication change is ordered for resident.  

Contact the parent/guardian within the same business day whenever possible but never more than 24 hours post appointment, to discuss medical appointments, review documentation and instructions, Medication changes, Illnesses or injuries requiring transport to Urgent Care or Emergency Department, Illness that requires a medical appointment or isolation/quarantine or injury that requires medical attention 

Must be able to perform physical activity such as extensive walking, lifting of patients, stretching and bending 

Report signs of abuse, neglect, and exploitation and sexually provocative behavior and implement professional response, reporting and documenting procedures 

Adhere to professional and ethical standards of conduct outlined in Breckenridge Village personnel polity manual including demonstrating respect for agency staff, residents, and community members and complying with required dress code at all times. 

Provide 1:1 (one to one) supervision for residents when medically necessary on our campus, in a hospital or other locations as needed. 

While providing 1:1 (one to one) supervision for residents, our LVN-PRN will act as an advocate on the resident’s behalf.  

 

Measurable Deliverables: 

1. Meet all state regulatory guidelines and standards that are applicable to this position, ensuring no deficiencies or concerns are found regarding medical care during annual fundamental surveys. 

2. Maintain resident physical and mental safety by making immediate report of any changes in resident health of behavior to the Nurse Manager and the QIDP with whom you collaborate 

3. Provide the highest level of resident care 

a. Daily review of progress notes and act as required, 

b. Daily review of incident reports and act as required 

c. Daily review of communication from residents, parents/guardians, QIDP or any other team member and act as required, 

d. Address each medical concern with urgency 

e. Treat resident medical needs as though you would for your own family 

f. In emergency situations, call 911 first 

4. Ensure proper time management of all daily, weekly, monthly, quarterly, and annual tasks 

5. Document all resident health information as close to real time as possible but not later than  24 hours 

6. Communication timeframes (during normal business hours): 

a. Phone Calls: Answer phone calls promptly when available. If unable to answer, return the call as soon as reasonably possible, with a goal of responding within one hour. 

b. Text Messages: Respond to text messages promptly when available. If unable to respond immediately, reply as soon as reasonably possible, with a goal of responding within one hour. 

c. Emails: Answer all emails with the goal of responding within one day.  

d. Respond sensitively and competently to the service population’s cultural and socio- economic characteristics 
b. Work collaboratively with other staff members, service providers, and professionals 
c. Communicate effectively in writing and verbally in English. 
d. Work in a fast-paced environment while maintaining control and professional composure at all times and making decisions based on the needs of the program and service population  

e. Maintain computer literacy required to meet the responsibilities of the position 
f. Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team

Credentials, Skills, and Abilities: 

1. Maintain current licenses and certificates required for the position including, but not limited to: 
a. Good standing as a Licensed Vocational Nurse with the state of Texas Board of Nursing 
b. Current BLS certifications and First Aid Certification 

Pass a pre-employment drug screen and random drug screens throughout employment. 

Valid Texas Driver’s License. 

Pass pre-employment and biennial criminal background checks. 

Must be able to maintain professional people skills, oral, written, listening, and non-verbal communication skills 

Demonstrate the ability to: 
a. Respond sensitively and competently to the service population’s cultural and socio- economic characteristics 
b. Work collaboratively with other staff members, service providers, and professionals 
c. Communicate effectively in writing and verbally in English.
