Nurse-Led Federally Qualified Health Center
Location: Philadelphia, Pennsylvania
Salary: $120,000–$150,000
Schedule: Full-time, Monday–Friday, plus one Saturday morning per month
Work Arrangement: Onsite
Reports to: Director of Primary Care
The Opportunity
Riam Recruiting is partnering with a respected, nurse-led Federally Qualified Health Center to identify a Primary Care Site Director.
This is a hands-on leadership opportunity for an experienced Nurse Practitioner who enjoys both patient care and team development. The Site Director will spend approximately three days per week seeing patients and two days leading site operations, clinical staff, quality, productivity, and patient flow.
The organization serves underserved Philadelphia communities and recently secured a $3.4 million federal health-center grant, supporting continued medical and behavioral-health services and long-term organizational stability.
Key Responsibilities
- Provide direct patient care approximately three days per week.
- Lead the daily clinical operations of an assigned primary care location.
- Supervise Nurse Practitioners and support nursing, medical assistant, and front-desk teams.
- Maintain effective patient flow, provider coverage, productivity, and site readiness.
- Support hiring, onboarding, performance reviews, coaching, and corrective action.
- Ensure providers and staff follow current clinical policies and procedures.
- Oversee medical supplies, vaccines, inventory, and other clinical needs.
- Collaborate with the Director of Primary Care and administrative leadership on staffing, coverage, and performance concerns.
- Promote teamwork and coordination with behavioral health, outreach, and other departments.
- Support quality-improvement initiatives and complete required chart audits.
Qualifications
- Master’s degree from an accredited Nurse Practitioner program
- At least five years of Nurse Practitioner experience
- Current Pennsylvania RN and advanced-practice nursing licenses
- Current national advanced-practice certification, or certification in progress
- Demonstrated clinical, leadership, and management abilities
- Experience supervising providers or multidisciplinary clinical teams
- Current CPR certification
- Ability to complete required background screenings and credentialing
- Reliable transportation and willingness to work onsite
Experience working in an FQHC, community health center, integrated-care environment, or with underserved populations is highly valued.

Compensation and Benefits
Compensation will be based on experience and qualifications.
The organization offers a comprehensive benefits package and participates in HRSA and Pennsylvania loan-repayment programs, subject to eligibility. Credentialing generally takes approximately three to four weeks, although payer enrollment may continue after the start date.
