Position Overview:
We are seeking a Licensed Practical Nurse to join a mission-driven non-profit organization operating federally qualified health centers. This role focuses on ensuring healthcare access for all through primary care, behavioral health, and health education services.

Qualifications:
- Certificate from an accredited practical nurse program.
- Current NY State LPN license and registration.
- 1–2 years in an ambulatory setting preferred.
- Venipuncture, medication administration, immunizations, and basic computer proficiency, including typing.
- Strong clinical, communication, interpersonal, and organizational skills; bilingual Spanish preferred.
- Demonstrated ability to organize care and activities of multiple patients.
Key Responsibilities:
- Administer medications, treatments, and wound care under RN supervision.
- Monitor and document patient conditions, vital signs, and progress.
- Assist with care plan implementation and collaborate with RNs, therapists, and physicians.
- Educate patients and families on post-treatment care, medications, and recovery goals.
- Maintain accurate clinical records, including medication logs and shift reports.
- Ensure compliance with infection control, safety protocols, and facility policies.
- Provide emotional support and advocate for the patient's needs during rehabilitation.
