Physical Therapy Assistant (PTA) - Home Care

MercyOne Home Care is seeking a compassionate and motivated Physical Therapy Assistant (PTA) to join our home health team in Mason City. In this role, you will provide hands-on physical therapy services to patients in their homes under the direction and supervision of a licensed Physical Therapist. You'll help patients improve mobility, restore function, and maintain independence while building meaningful relationships with patients and their families.

What You'll Do

• Deliver physical therapy services according to physician orders and the Physical Therapist's plan of care.
• Implement treatment programs developed by the supervising Physical Therapist.
• Provide direct patient care and therapeutic interventions in the home setting.
• Educate patients and caregivers on exercises, mobility techniques, and the use of adaptive equipment such as wheelchairs, braces, and other assistive devices.
• Monitor patient progress and report changes in condition to the Physical Therapist, physician, RN, or leadership team.
• Document treatments, observations, and patient responses accurately and timely using the electronic medical record system.
• Collaborate closely with therapists, nurses, and other care team members to ensure high-quality patient care.
• Participate in care conferences and patient care planning meetings as appropriate.
• Provide ongoing encouragement and support to patients and families throughout their care journey.

Qualifications

Required

• Associate degree from a Physical Therapist Assistant program approved by the American Physical Therapy Association (APTA).
• Minimum of six months of experience working under the supervision of a qualified Physical Therapist.
• Current valid driver's license and reliable transportation.
• Ability to work independently with minimal supervision.
• Strong communication and documentation skills.

Preferred

• Previous home health experience.
