Occupational Therapist – Help Patients Rediscover Independence at Home

Home Healthcare, Hospice & Community Services is seeking an Occupational Therapist to help individuals regain independence, adapt to changes in their abilities, and safely participate in the activities that matter most to them - all within the comfort and familiarity of their own homes.

As an Occupational Therapist with HCS, you’ll have the opportunity to look beyond the diagnosis and understand each patient’s goals, routines, environment, and support system. You’ll develop individualized plans of care, provide skilled interventions and education, and collaborate with patients, families, physicians, and an interdisciplinary team to help patients achieve meaningful, realistic outcomes.

If you’re an OT who values autonomy, relationship-based care, and the opportunity to see the real-world impact of your work, we’d love to connect.

  • A Holistic Approach: Consider the physical, psychosocial, environmental, cultural, and personal factors that influence each patient’s ability to thrive at home.
  • Interdisciplinary Collaboration: Partner with physicians, nurses, therapists, aides, families, and community resources to coordinate care and support successful outcomes.
  • Education That Makes a Difference: Help patients and caregivers build the knowledge, skills, and confidence they need to safely manage care and maximize independence.
