Summary:

Purchase District Health Department is seeking a knowledgeable and highly motivated individual to join the Rural Community Hubs for Chronic Care Innovation (RCH) program in the Purchase Area Development District.

This full time, term limited role is part of the Rural Health Transformation–RCH program, with Purchase District Health Department serving as the RCH Lead Organization for Purchase Area Development District and the Foundation for a Healthy Kentucky serving as statewide, neutral convening and implementation partner for the RCH program. The Rural Community Hub (RCH) Operations Lead is responsible for operational management, partner engagement, financial oversight, and technical assistance within the Purchase Area Development District Hub. This role supports a statewide, multi-sector collaborative initiative to improve the health and wellbeing of rural Kentuckians.

The RCH Operations Lead will report to the Purchase District Health Department and work closely with the FHKY RCH Coordinator to help establish and scale Hub operations by developing foundational infrastructure, coordinating partners, supporting program implementation, and maintaining effective operational systems. As the initiative matures, the role will focus on optimizing workflows, supporting advancing ADD-level strategies that align local priorities with state goals.

Success in this position requires strong project management skills, experience coordinating multiple stakeholders, and the ability to translate strategy into effective operational implementation.

This investment will empower Kentucky to launch and implement its Rural Health Transformation Plan (RHTP) — a community-driven strategy to expand access and improve health outcomes for rural residents across the Commonwealth. This project is 100% funded by the Centers for Medicare and Medicaid Services of the U.S. Department of Health and Human Services (HHS) for a total of $212.9 million in budget year 1 with 100% by CMS/HHS. The RHTF grant spans five budget periods aligned with federal fiscal years (FY2026–FY2030). Funding is disbursed annually by CMS based on achievement of performance metrics. Description:

Responsibilities:

  • Manage day-to-day operational activities of the Rural Community Hub, driving alignment with program goals, grant requirements, and local community needs
  • Support budgeting, expense tracking, procurements, and financial reporting processes to drive accountability and transparency
  • Support Purchase District Health Department with fulfilling its responsibilities as a passthrough entity in accordance with 2 CFR 200
  • Guide Hub partner organizations on federal grant compliance considerations (e.g., cost allowability, payment processes)
  • Oversee vendor and contractor management for the Hub, including procurement, contracting, monitoring, and performance oversight
  • Develop and maintain systems for accurate data collection, performance tracking, and timely reporting of Hub activities and outcomes
  • Coordinate technical assistance and capacity-building support for Hub partners, helping organizations access tools, resources, and expertise needed for successful implementation
  • Monitor progress against project plans, deliverables, milestones, and performance measures; proactively identify and resolve operational challenges
  • Partner with Hub leads and subject matter experts to embed operational leading practices into program design and implementation
  • Establish and maintain systems, processes, and workflows that enable effective Hub operations and partner coordination

Relationship Management and Collaboration:

  • Develop and maintain strong working relationships with local organizations, service providers, employers, community leaders, RHT funders, and other stakeholders
  • Support stakeholder engagement efforts and serve as a key operational liaison between Hub partners and statewide initiative leadership

Operational Efficiency and Improvement:

  • Identify opportunities to improve operational efficiency, strengthen partner support, and enhance program implementation
  • Support the deployment of program-wide operational enhancements and/or processes
  • Gather and assess feedback from partners and stakeholders to inform ongoing operational improvements
  • Stay informed on leading practices related to rural health transformation, community partnerships, and operational management, incorporating relevant lessons into Hub operations

Your Background:

  • Bachelor's degree in nonprofit management, public health, social work, business administration, healthcare administration, or a related field
  • 4+ years of experience in program coordination, operations management, community engagement, partnership development, or related fields
  • Experience managing multiple priorities, timelines, and stakeholder relationships simultaneously
  • Demonstrated ability to coordinate projects and support implementation across multiple organizations or sectors
  • Excellent written and verbal communication skills
  • Strong organizational, facilitation, and problem-solving abilities
  • Ability to travel throughout ADD region for meetings, events, and partner engagement activities
  • Deep commitment to improving health outcomes and quality of life for Kentuckians
  • Experience working within rural communities and/or Purchase Area Development District communities preferred

  • Experience working with collective impact initiatives, multisector collaborations, community-based organizations, or place-based change efforts
  • Experience supporting grants, contracts, budgeting, reporting, and performance measurement activities
  • Familiarity with healthcare, public health, social services, education, workforce development, or community development systems
  • Knowledge of population health principles and rural community health challenges

  • Data-Driven Decision Making: Utilize qualitative and quantitative data to support operational planning, performance monitoring, and continuous improvement.
  • Communication and Facilitation: Lead meetings, coordinate stakeholder engagement, and communicate effectively with technical and non-technical audiences.

Salary Range: $29-$32/hour based on qualifications and experience

The Purchase District Health Department (PDHD) is a public health agency with jurisdiction in five counties (Ballard, Carlisle, Fulton, Hickman, and McCracken) in the Purchase area. The mission of PDHD is to provide environmental, preventive, curative, and health maintenance services to area citizens.
