Description

The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco.  The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve.

We are currently looking for qualified candidates for our Registered Dietitian role. This position reports to the Wellness Director and is a Regular, Full Time, Exempt role with a starting annual range of $85,937 to $94,572 with full benefits. 

Primary Objective:

The Registered Dietitian is a critical care team member responsible for assessing the nutritional status of patients, tracking and following-up with patients with chronic diseases, and maintaining a panel of Adult Medicine patients enrolled in Diabetes Care Management. This role closely collaborates with members of the Adult Medicine Care Teams.

Essential Functions/Responsibilities:

Direct Patient Care (70%)

  • Collaborate with providers, nurses, behavioral health staff, and other Care Team members to identify patients who would benefit from Medical Nutrition Therapy (MNT), based on clinical indicators, screening results, and provider referrals.
  • Conduct comprehensive nutrition assessments, including medical history, diet history, social determinants of health, laboratory data, anthropometric measurements, and medication review.
  • Develop and implement individualized MNT care plans in partnership with the patient, incorporating cultural food preferences, health literacy, and readiness to change.
  • Provide ongoing nutrition counseling and lifestyle coaching to address conditions such as diabetes, hypertension, hyperlipidemia, obesity, renal disease, gastrointestinal disorders, and prenatal/postpartum needs.
  • Monitor patient progress toward nutrition and lifestyle goals, adjusting interventions as clinically indicated.
  • Document all encounters, assessments, interventions, and outcomes in the Electronic Health Record (EHR) according to organizational and regulatory standards.
  • Co-facilitate shared medical visits and chronic disease management programs, providing nutrition expertise and patient education.
  • Provide MNT one day per week at Clínica Esperanza, working closely with the HIV Care Team to address the nutritional needs of patients living with HIV/AIDS, including those with comorbidities such as diabetes, hypertension, and food insecurity.
  • Provide MNT and nutrition counseling at the Mission Bernal clinic, supporting patients across the lifespan with chronic disease management (diabetes, hypertension, obesity, hyperlipidemia), and preventive nutrition care in collaboration with the primary care team.
  • Use population health management tools and patient registries for proactive outreach, follow-up, and care coordination.
  • Connect patients to community nutrition and food security resources (e.g., WIC, EatSF vouchers, Project Open Hand, CalFresh, food pantries) and serve as a liaison with these agencies.
  • Represent the organization at community coalitions, health fairs, and partner networks related to nutrition, diabetes prevention, maternal health, and other chronic disease prevention initiatives.
Group Health Education (20%)
  • Design, coordinate, and facilitate group MNT sessions, preventive health classes, and chronic disease self-management workshops.
  • Develop culturally and linguistically appropriate nutrition education materials and curricula for group settings.
  • Evaluate the effectiveness of group interventions through patient feedback, attendance tracking, and outcome measures.
  • Support health promotion campaigns and community-based nutrition initiatives.
Administrative (10%)
  • Develop and update clinical nutrition protocols, workflows, and staff training modules to integrate MNT into primary care.
  • Participate in care team huddles, case conferences, and multidisciplinary rounds to provide nutrition input and care coordination.
  • Serve as a subject matter expert on nutrition for staff education, provider support, and program planning.
  • Collaborate with the Quality Improvement Committee to identify, implement, and evaluate initiatives aimed at improving nutrition-related outcomes (e.g., A1C, blood pressure, weight management)
  • Maintain active knowledge of evidence-based nutrition guidelines, emerging research, and regulatory changes affecting MNT.
  • Participate in the Patient Advisory Committee and incorporate patient feedback into service improvements.
  • Perform other duties as assigned by the Supervisor.
    As part of the Nutrition Department, plan activities and events for National Nutrition Month, Diabetes Awareness Month, etc. 

    Participates in monthly department meetings.

MINIMUM QUALIFICATIONS
  • Bachelor’s or master’s degree in nutrition/Dietetics from an ACEND-accredited program required.
  • Current registration as a Registered Dietitian Nutritionist (RDN) by the Commission on Dietetic Registration (CDR) required or eligible to sit for the RD registration examination and passes exam within 6 months of completing ACEND-accredited supervised practice program.
  • Bilingual in English and Spanish.
  • Current Basic Life Support (BLS) certificate.
  • Two years of experience providing MNT and patient-centered nutrition counseling in clinical, community health, or ambulatory care settings.
  • Demonstrated experience designing and facilitating group health education or self-management programs
  • Knowledge and application of evidence-based MNT guidelines for chronic disease prevention and management.
  • Basic to intermediate proficiency with Microsoft Office Suite (Word, Excel, Outlook)
  • Ability to maintain the highest level of confidentiality in compliance with HIPAA and organizational policies.
PREFERRED QUALIFICATIONS
  • Certified Diabetes Care and Education Specialist (CDCES) or Board-Certified Specialist in relevant practice area (e.g., renal, pediatrics).
  • Experience with electronic health records.
  • Experience with population health registries.
  • Experience in primary care, ambulatory care, or community health centers.
  • Experience with quality improvement initiatives and data-driven practice improvement.
  • Project management and program development experience.
  • Knowledge of principles of motivational interviewing and behavioral change theories.
