The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time).

Position Overview

Capital Health is seeking a compassionate and experienced Nurse Practitioner (NP) to join our PACE (Program of All-Inclusive Care for the Elderly) team. The Nurse Practitioner will provide comprehensive, patient-centered care to older adults with complex medical and functional needs, helping participants maintain their health, independence, and quality of life while remaining safely in the community.

We are seeking a Nurse Practitioner who is passionate about geriatric medicine, collaborative care, and helping older adults maintain their independence and quality of life while contributing to the continued growth of Capital Health’s PACE program.

Opportunity Details

  • Provide comprehensive primary and preventive care to PACE participants in collaboration with physicians and the interdisciplinary care team.
  • Perform initial and ongoing health assessments, physical examinations, and management of acute and chronic medical conditions.
  • Develop, implement, and evaluate individualized plans of care based on each participant’s medical, functional, psychosocial, and long-term care needs.
  • Diagnose and treat illnesses and injuries and prescribe medications and treatments within scope of practice.
  • Monitor and manage chronic conditions commonly affecting older adults, including diabetes, hypertension, cardiovascular disease, pulmonary conditions, and other complex medical needs.
  • Order and interpret laboratory testing, diagnostic studies, and other appropriate clinical evaluations.
  • Provide medication management and reconciliation, with attention to polypharmacy and medication safety in older adults.
  • Collaborate closely with the PACE interdisciplinary team, including physicians, nurses, social workers, therapists, dietitians, behavioral health professionals, and other members of the care team.
  • Participate in interdisciplinary team meetings and contribute to comprehensive care planning.
  • Coordinate care across settings, including the PACE center, participant homes, hospitals, skilled nursing facilities, rehabilitation facilities, and specialty practices as appropriate.
  • Provide education and counseling to participants and their families regarding diagnoses, medications, treatment plans, preventive care, and healthy aging.
  • Support transitions of care following hospitalization, emergency department visits, or rehabilitation stays.
  • Maintain timely and accurate documentation in the electronic medical record and comply with applicable regulatory and organizational requirements.
  • Participate in quality improvement, patient safety, and population health initiatives supporting the PACE program.

Qualifications

  • Master’s or Doctoral degree from an accredited Nurse Practitioner program.
  • Current or eligible New Jersey Advanced Practice Nurse (APN) license.
  • National board certification as a Nurse Practitioner.
  • Current New Jersey RN license as required.
  • Current BLS certification.
  • Prescriptive authority and DEA registration as required for the position.
  • Experience in geriatrics, primary care, internal medicine, home-based care, long-term care, or complex care management strongly preferred.
  • Experience caring for medically complex older adults preferred.

The Program of All-Inclusive Care for the Elderly (PACE) provides comprehensive medical, behavioral, rehabilitative, social, and long-term care services to older adults who meet nursing-home-level care criteria but are able to live safely in the community with appropriate support.

The PACE model emphasizes coordinated, interdisciplinary care, allowing providers to develop meaningful relationships with participants while addressing the full spectrum of their healthcare and social needs.
