Hospice RN Case Manager – Kingsville/Corpus Christi, TX

You will support hospice patients in their homes and care settings across South Texas, providing compassionate end-of-life nursing care with the backing of an interdisciplinary hospice team.
Opportunity
A community-based hospice provider in South Texas is hiring a full-time Hospice RN Case Manager to manage a patient caseload across the Kingsville / Corpus Christi area and nearby communities. This is a field-based role for an RN who is comfortable traveling locally, assessing patients independently, coordinating care plans, and communicating clearly with patients, families, physicians, and the hospice team.
This is a strong fit for a hospice RN case manager, home health RN, field nurse, or experienced clinical RN who wants to move into meaningful end-of-life care.
Location & Work Model
- Field-based role serving patients across the Kingsville / Corpus Christi, TX area and nearby communities
- Candidates should live within or near the territory, or be able to reliably travel throughout the assigned service area
- Daily local travel for hospice field visits in homes, facilities, and community settings
- Full-time schedule with participation in a shared on-call rotation
Compensation
The expected total compensation range is USD 80,000–83,000 Monthly.
What You Will Own
Hospice Case Management
- Manage a hospice patient caseload throughout the assigned Kingsville / Corpus Christi territory
- Conduct initial and ongoing nursing assessments
- Create, update, and carry out individualized hospice plans of care
- Monitor patient condition, symptoms, medication needs, and changes in disease progression
- Provide nursing support focused on comfort, dignity, and quality of life
Patient & Family Support
- Educate patients and families on symptom management, medication management, disease progression, and end-of-life expectations
- Communicate changes in patient condition clearly and promptly with families, physicians, and the hospice team
- Support families with calm, practical, compassionate guidance during the hospice journey
- Help patients receive reliable care in the setting that best supports their comfort and goals
Interdisciplinary Team Coordination
- Collaborate with social workers, chaplains, aides, medical directors, physicians, and clinical leadership
- Participate in care planning and interdisciplinary team coordination
- Escalate clinical concerns appropriately and support timely updates to the plan of care
- Maintain accurate, complete, and timely documentation in the EMR
Field-Based Care
- Travel daily within the assigned service area
- Provide care in patient homes, facilities, and other community settings
- Participate in shared on-call coverage and help support urgent after-hours patient needs when scheduled
What Makes You a Strong Fit
- Active, unrestricted RN license
- Comfortable working full-time in a field-based hospice role
- Reliable transportation, valid driver’s license, and ability to travel across the assigned territory

Preferred Experience
- Hospice RN Case Manager experience
- Home health field nursing experience
- Experience managing a patient caseload

- Experience communicating with families during serious illness or end-of-life care
- Stable work history, ideally averaging longer tenure across recent roles
Transferable Backgrounds Welcome
Hospice experience is preferred, but strong transferable nursing backgrounds will be considered. You may be a strong fit if you have experience in:
- Home health nursing
- ER or ICU nursing
- Skilled nursing facility, nursing home, or long-term care nursing
- Field-based care or case management
- Serious-illness, terminal-illness, or high-acuity patient care
