Description
This is a hands-on position where no two days are exactly alike. You may spend part of your day communicating with families, reviewing a client's needs, coordinating caregivers, completing a home visit, updating a care plan, and helping resolve an unexpected scheduling or care issue.
- Build strong, trusting relationships with clients and their families.
- Complete client intakes, assessments, and ongoing care evaluations.
- Develop and update individualized care plans based on each client's needs.
- Communicate regularly with clients, families, caregivers, healthcare professionals, and other involved parties.
- Conduct in-home visits to check on clients and ensure quality of care.
- Monitor client needs and identify changes that may require adjustments to services.
- Coordinate with the scheduling team to help ensure appropriate caregiver coverage.
- Support, train, and communicate with caregivers regarding client needs and expectations.
- Address client and family concerns promptly and professionally.
- Maintain accurate client records, care documentation, and follow-up notes.
- Identify gaps in care and work with the team to find practical solutions.
- Help ensure services are provided according to company policies and applicable regulations.
- Participate in quality improvement and help maintain Hummingbird's high standards of care.

- Experience working with older adults or individuals who require assistance at home is strongly preferred.
- Knowledge of basic caregiving practices and home care standards.
- Strong organizational, communication, and time-management skills.
- Ability to maintain confidentiality and handle sensitive client information professionally.
- Proficiency with email, Microsoft Office/Google Workspace, scheduling software, and electronic documentation systems.
- High school diploma or equivalent required.
- Associate or bachelor's degree in nursing, social work, healthcare administration, or a related field is preferred.
- Nursing, social services, healthcare administration, or supervisory experience is a plus.
