Summary:


The BHH Nurse is responsible for assisting in patient care management and the delivery of health care to individuals with mental health, substance use disorders and primary care needs. 


Essential Duties:

  • Conducts outreach and engagement of referrals (by phone or in person), makes appropriate referrals and case assignment.
  • Conducts initial and ongoing health assessments of referred clients (ie. BHH, DMHAS, Bio/Psych/Social, etc) related to program requirements.
  • Carry a caseload and/or provide assistance to Community Support Services (CSS)/BHH staff with caseload. Assist in scheduling treatment/appointments and navigating health care needs.
  • Performs care coordination and chart reconciliation for clients. Follows up with referral, test results, medication, incomplete orders, etc.
  • May give injections, draw blood, deliver medications; as well as other patient care services directed by the physicians.
  • Coordination with internal and external providers (hospitals, clinics, family/supports, etc).
  • Provides psychoeducation to CSS and clients around health care needs/topics; may facilitate Psychoeducation groups to both colleagues and clients.
  • Documentation in EHR of client coordination and contact. May assist in service planning development and interventions.
  • Attends internal and external BHH required meetings and training.
  • May compile and condense technical and statistical data for reports and records.
  • Preforms related duties as required by CSS program needs.

*All agency staff are required to attend all mandatory department/agency meetings and trainings*


Schedule: 


Monday - Friday, 8:00 AM - 4:30 PM


Requirements

Education &/Or Responsibilities:


Graduation from an accredited nursing program. Current State of Connecticut LPN or RN licensure required. One-year medical experience in a physician’s office required.
