The Home Health Nurse Residency program is a post NCLEX position tailored for newly graduated nurses from an accredited institution to complete a twelve month structured program designed to transition the new graduate nurse successfully to the role of a proficient Home Health RN Case Manager. The Home Health Nurse Resident will have direct oversight by the Home Health RN Nurse Mentor to ensure that all assignments and responsibilities are competently completed. The Nurse Resident will shadow and be exposed to the various clinical environments and other disciplines working together to create a patients careteam.
