Position Summary
- Responsible for supervising the implementation of the Tobacco Disparities Program Grant. This is a grant-funded position that reports to the System Director of Community Health.
- Grant Summary: The purpose of the Community-Based Reducing Tobacco- Related Disparities Grant is to engage regional and local coalitions, local health departments, healthcare systems, community-based organizations, and other partners to assess, plan, and implement evidence-based tobacco control interventions to reduce geographic tobacco-related disparities in the sixteen southernmost counties of Illinois. Deliverables under this grant are required by and will be aligned with the Centers for Disease Control and Prevention (CDC), Office on Smoking and Health, National Tobacco Control Program, community- based disparities initiative. The target populations include individuals who use tobacco products and have one or more chronic diseases, low socioeconomic status, low educational attainment, a behavioral health diagnosis, military/veteran status, or pregnancy.

- Technical Experience:
- 3 years experience in program development and management

- Administrative skills in data and survey collection
- Must possess effective oral and written skills for large groups or individual presentations
- Performance management and grant implementation knowledge and experience required.
- Advanced computer knowledge and skills required

Role Specific Responsibilities
- Work with additional grant staff as a team in conjunction with System Director for Community Health and SIH Grants Department;
- Oversees Illinois Department of Public Health Tobacco Disparities Program grant and accomplishes grant goals and objectives, as able
- Responsible to ensure Tobacco Disparities Project is being implemented in compliance with grant requirements, as well as established SIH policies and procedures.
- Convenes a regional coalition, with appropriate subcommittees or workgroups, to address tobacco disparities in the geographic region. The coalition must include, but not be limited to, representatives of community stakeholder organizations, community leaders, local health departments, health care systems, public and private multi-unit housing, local business community/chamber of commerce, faith-based organizations, youth-serving organizations, and education.
- Organizes and meets with the regional partnership monthly, with subcommittees meeting during intermediate intervals.
- Collaborates with the regional coalition to develop a four-year strategic plan, subject to IDPH approval, which includes data collection, evaluation, health communication, and sustainability plans, capacity building and training for community organizations, and culturally appropriate policy, systems, and environmental (PSE) strategies and activities which seek to improve health equity and reduce tobacco-related disparities.
- Evaluates the process, outcomes, and ROI of the regional collaborations and correlating initiatives; Drives the strategic plan created by the regional coalition/partnership
- Upon IDPH approval of the strategic plan:
- Develops a request for application for competitive subgrants to be awarded to community organizations to implement approved interventions from the strategic plan.
- Executes a minimum of 10 subgrants in the lead agency region with organizations with demonstrated experience, readiness, and capacity to provide tobacco prevention and control interventions and strategies to the target population.
- Ensures subgrantee work plan, budget, and periodic fiscal and programmatic reporting requirements adhere to the same guidelines that the lead agency is subject to under the Community-Based Reducing Tobacco-Related Disparities Grant. Subcontract and Partnership Management:
- Ensure meticulous documentation and record-keeping subcontracts, monthly communication, site visits/meetings, quarterly progress reports, and financial reports associated with subcontracts and partnerships.
- Conduct evaluation of subgrants as specified in strategic plan/subcontracts.
- Conduct or update a regional community health needs assessment which may include new surveys, demographic characteristics, health status, community profile, existing tobacco control policies, and available health care systems in the designated lead agency region.
- Participates in and coordinates the budgeting process for the Tobacco Disparities grant.
- Oversee record-keeping of subcontracts in coordination with Tobacco Disparities Grant Coordinator.
- Maintains regular communication with the grant funder and serve as their primary point of contact.
- Electronic Record Keeping: Maintain accurate and up-to-date electronic records of all program materials and implement efficient systems for easy retrieval and organization of information.
- Submits annual performance measurements, including but not limited to, number and reach of: activities to promote and support evidence-based quit support services; healthcare systems which incorporate tobacco use and dependence screening with all patients; media placements including paid, earned, social or digital media targeting populations experiencing tobacco-related disparities in the region; and local tobacco-free policies or ordinances supported, introduced, adopted, or implemented.
- Supervises the development, planning, management, and reporting process for Tobacco Disparities grant funding.
- Responsible for coordinating approval of expenditure reports through the Grants Department.
- Responsible for reporting on project activities and for reporting all other operational and administrative issues.
- Responsible for routing any and all subcontracts through the SIH Grants Department.
- Responsible for completing wage certifications of Tobacco Disparities grant-funded staff to the Grants Department.
- Facilitate media exposure and education outreach for the Illinois Department of Public Health Tobacco Disparities Program grant. Ensure items meet grant requirements; tagline, logo and approval of IDPH staff.
- Plan, implement, and evaluate mass-reach health communications for the region, including paid traditional media, digital and social media, and earned media, targeting the priority populations, to increase awareness of tobacco-related disparities, coalition initiatives, cessation resources, and, the dangers of secondhand smoke exposure.
- Negotiates contracts with local media
- Coordinate with tobacco disparities research team(s) to create fact sheets and bulleted, media-ready content.
- Coordinate outreach activities aimed at promoting the tobacco grant/partnership, tobacco cessation programs and raising awareness.
- Distribution and tracking of education material distribution.
