Homelake Veterans Community Living Center
The Colorado State Veterans Center at Homelake is a State-owned and operated skilled nursing facility and domiciliary dedicated to serving those who have served us.  We strive to provide excellent customer service and to respect and honor resident choices regarding bathing, rising, bedtimes, and other personal decisions.  Our philosophy is to provide residents a home where life and love abound.   
While in this position, you are required to work at your assigned work location during your scheduled work times. 
Currently recruiting for the following schedules: 
Tuesday - Saturday, 10:00 pm - 6:30 pm 
and 
Thursday, 2:00 pm - 10:30 pm; Friday - Monday, 10:00 pm - 6:30 pm 
Please note: Shifts may vary depending on business needs. Work arrangements are subject to change at any time.

The purpose of this position is to provide nursing oversight, administrative oversight, and lead work over nursing personnel of three or more FTEs on a continuous basis, as well as clinical supervision of all nursing staff during a specific shift. Provide direction to all direct care nursing staff on assigned shift(s). Serve as a resource and mentor to all nursing staff on assigned shift. Promote and maintain nursing staff efficiency. This job provides professional medical care to people living in the facility. This includes checking residents' health and giving them specialized nursing care that follows professional standards, laws, regulations, and the facility's policies and procedures. The nurse provides regular and special nursing care, such as medication and treatments, and documents the care provided in the residents' electronic medical records (EHR).

Additionally, as a Nurse II, you can expect to: 
  • Perform admission and daily assessments, develop individualized care plans, and monitor for critical health changes and promptly notify physicians.
  • Safely administer routine and specialized interventions (including IV therapy, venipuncture, and enteral nutrition) while monitoring for medication effectiveness and interactions.
  • Direct, delegate, and prioritize daily assignments for nursing staff (RN, LPN, CNA) based on facility workload and resident needs.
  • Orient new employees, provide regular coaching and performance feedback, and ensure required staff competencies are completed and evaluated.
  • Maintain accurate, timely resident records in the EHR, including comprehensive assessments, progress notes, and medication/treatment administration records (MAR/TAR).
  • Enforce strict adherence to VCLC, State of Colorado, and VA regulations, while leading and educating staff on essential infection control precautions.
  • Actively participate in continuous quality improvement teams to establish best practices and resolve clinical concerns including pharmacy and medication work groups.
  • Collaborate with the Director of Nursing and House Supervisors to guarantee top-tier unit care, promptly communicating policy and procedural changes to your team.
  • Operate a State vehicle to conduct off-site assessments or safely transport residents as required.
  • Maintain clinical expertise and regulatory knowledge through ongoing education, workshops, webinars, and relevant literature reviews.
  • Perform other duties as assigned
Special Qualifications:
  • Required to possess a current BLS or CPR card and maintain a Colorado RN license in good standing

and

Minimum Qualifications:

  • Current, valid licensure as a Registered Nurse from the Colorado Board of Nursing or any state participating in the Enhanced Nurse Licensure Compact (eNLC)
and

Experience:

  • Two (2) years of professional experience in an occupational field related to the work assigned to the position

Substitutions:

  • Appropriate education will substitute for up to two (2) years of the required experience on a year-for-year basis

  • FULL BACKGROUND CHECK:  This position will have direct contact with vulnerable persons (as defined by C.R.S.27-90-111: "Direct contact" means providing face-to-face care, training, supervision, counseling, consultation, or medication assistance to vulnerable persons, regardless of the level of supervision of the department employee. "Direct contact" may include positions in which persons have access to or unsupervised time with clients or patients, including but not limited to maintenance personnel, housekeeping staff, kitchen staff, and security personnel.) 
    • Full background checks include:
      • CBI & FBI fingerprint check, Colorado Court Database check, Medicare Fraud Database, TRAILS Database check.
      • This position will also provide direct care to vulnerable adults requiring a CAPS check (as defined by C.R.S. 26-3.1-101, position provides services & support care to at-risk/vulnerable adults including case management, clergy, physical or mental health services, safety or welfare services)This position requires State of Colorado residency at the time of application (unless otherwise identified in the posting), and residency within the state throughout the duration of employment in this position.

  • Must possess a valid, non-restricted Colorado Driver’s License 
  • Must be willing to obtain vaccinations, as required by the State and facility
  • May be required to travel/transport residents in state vehicles
  • Shift Work: weekends, holidays, evenings, nights, and days
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must:
    • Disclose that information on the application.

    • Explain why the prior termination or resignation should not disqualify you from the current position.

    • Provide your employee number from your prior State employment.  Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination, and failure to provide this information will disqualify the applicant from future State employment with CDHS.


CDHS Selection Process Explanation
Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications. Be sure your application specifically addresses your qualifications, experience, work products, and accomplishments as they relate to the position and minimum requirements.

After minimum qualification screening, the comparative analysis process will involve reviewing and rating of all the information you submit. The comparative analysis step may also include your results from standardized testing.
