Description
- Conduct medical evaluations, treatments, and consultations for facility residents.
- Proactively manage chronic conditions to minimize disease progression, lessen the risk of major acute events, alleviate side effects, and enhance quality of life for residents.
- Address and manage acute medical situations as they arise with urgency and compassion.
- Collaborate effectively with facility staff and the Medical Director.
- Issue medical orders for prescriptions, medication administration, tests, and referrals.
- Communicate with residents' families when necessary.
- Promptly document patient interactions within 24 hours using dictation.
- Review test results and ensure patient notification.
- Deliver patient education as required.
Qualifications and Experience:
- Must hold an active Advanced Level Practitioner license, open to both Nurse Practitioners (NP) and Physician Assistants (PA).
- Capability to independently identify patients for care, set personal schedules, and establish effective relationships with facility staff.
- Proficiency in working autonomously and making independent decisions.
- Experience in an institutional inpatient setting is preferred.
- Prior work in skilled nursing and/or long-term care facilities is advantageous.
- Experience with geriatric patients is desirable.
- Experience in other settings including traditional outpatient clinics, urgent care clinics, and other specialty clinics will also be considered.
