Now Hiring: Full-Time MDS RN
Diablo Post Acute is seeking an experienced and compassionate Registered Nurse (RN) – Minimum Data Set (MDS) Coordinator to join our dedicated team!
Full-Time MDS RN
$100,000–$120,000 per year

Responsibilities
Coordinate and complete MDS assessments in accordance with state and federal regulations.
Ensure accurate and timely completion of resident assessments and care plans.
Collaborate with the interdisciplinary team to optimize resident outcomes.
Maintain compliance with CMS guidelines and documentation requirements.
Participate in quality improvement initiatives and survey readiness.
Qualifications
Current RN license in California.
Previous MDS experience in a skilled nursing facility preferred.
Strong knowledge of Medicare, Medicaid, and CMS regulations.
Excellent organizational, communication, and documentation skills.
Ability to work collaboratively in a fast-paced environment.
