Responsibilities

The PPS/Referral Coordinator LPN facilitates admissions to the Rehabilitation Unit. The position involves acting as liaison between the physician, staff and rehabilitation unit to facilitate the admissions process for internal and external referrals. PPS/Referral Coordinator LPN completes FIM scores day four and on discharge. Responsible for staff education and FIM recertification. Transmits data to CMS for reimbursement, maintains transmission log and coordinates this with coders and patient accounting who submits the UB04. Remains current with CMS rules and regulations. In collaboration with the healthcare team, Rehab Director and under the supervision of a Registered Nurse (RN), the LPN works to maintain safety, provide patient comfort and care and improve the overall patient experience in the physical rehabilitation setting

Education and Work Experience

 

1. Current and valid Michigan Licensed Practical Nurse (LPN) licensure required.2. BCLS certification.3. A minimum of two years clinical experience in the chronic or acute care rehabilitation preferred.
