The Opportunity
As a Clinical Support Registered Nurse, you will provide clinical support to clients and caregivers in their homes and in the community. You will focus on quality, health education, care coordination, chronic condition management, and connecting clients with appropriate healthcare and community resources, while ensuring that all client visits and documentation meet MassHealth program regulations and insurer requirements.
In this role, you will advocate for clients, promote safe and effective care at home, and support ongoing care planning through direct engagement and coordination with interdisciplinary teams.
Key Responsibilities
- Supports scheduled monthly client visits to provide caregiver education, monitor client status, and assess psychosocial and environmental changes
- Support semi-annual care plan reviews, updating goals and interventions in coordination with nursing and interdisciplinary teams
- Provide education on health maintenance, preventative care, chronic disease management, and medication adherence
- Support clients and caregivers in navigating healthcare systems and accessing community-based services and resources
- Participate in multidisciplinary care planning, case reviews, and discharge planning
- Identify and address changes in psychosocial status or care needs and escalate concerns as appropriate
- Collaborate with internal teams and external providers, including PCPs and specialists, to coordinate care
- Maintain accurate, timely, and compliant documentation in accordance with regulatory standards
- Communicate professionally with clients, caregivers, and healthcare partners in person and through phone and electronic platforms
- Participate in team meetings and contribute to ongoing care coordination efforts
- Ensure all client visits are scheduled, conducted, and documented in compliance with MassHealth program regulations and applicable insurer requirements
- Conduct quality checks and audits of client records, visit documentation, and care plans to verify accuracy, completeness, and regulatory compliance
- Facilitates MDT meetings in collaboration with management and care teams
- Perform additional duties within scope of practice as assigned

What we are looking for:
- Active Registered Nurse (RN) license in Massachusetts (required)
- Minimum of 2 years of recent experience working with older adults or individuals with disabilities in a clinical, home health, case management, or related setting
- Strong communication, interpersonal, and client engagement skills
- Experience with care coordination, patient education, or case management preferred
- Proficiency with electronic medical records and basic technology tools
- Valid driver’s license and reliable transportation required for travel to client homes and community settings
- Bilingual skills (English and a second language) strongly preferred
Work Environment and Physical Requirements
- Office-based work in a clean well-lit environment with fluctuating temperatures near others.
- Client-facing field staff and community liaisons require frequent travel to client homes or community settings; ability to drive safely; work in client homes may involve varying temperatures, odors, allergens, pets, and other environmental factors.
- Requires substantial periods of repetitive work utilizing a computer, monitor, keyboard, and mouse.
- Requires lifting and carrying equipment and supplies weighing up to 35 pounds; requires pushing and pulling equipment and supplies weighing up to 35 pounds; requires walking and standing; requires frequent sitting more than 75% of the workday; requires the ability to negotiate stairs; requires visual acuity and manual dexterity to operate equipment.
