Description:

This is a grant funded position. The Talent Acquisition Partner will be able to provide more details in the form of a Grant Funded Position Agreement.

Position Purpose 

Provides supervision, coaching, and direct support to a team of CHWs working to improve health outcomes and

eliminate racial, ethnic, and economic disparities in those outcomes. Plays an integral role in program management,

fostering growth of CHWs, ensuring quality of service delivery, and guiding progress towards intervention goals.

Responsible for the daily functions of a team of CHWs, including health system navigation, health education, patient

engagement and follow-up, resource advocacy, and outreach activities. Assists with program design, evaluation, and

evolution.

What you will do

  • Daily operations & the oversight of CHWs including hiring, training, professional development, performance evaluations, and onboarding/offboarding

  • Coordinate staff scheduling & assignment; review & approve administrative functions (time, payroll, expense)

  • Work in collaboration with department leadership to monitor program performance, quality improvement, and service delivery

  • Work internally to align programs, workflows, and referral processes

  • Provide leadership and guidance on best practice techniques to CHW staff

  • Implement departmental plans & priorities identified by accountable leaders

  • Assist with the design and evaluation of CHW interventions including the integration in clinical care

  • Promote CHW services within the health ministry and community through ministry and community events, speaking engagements, inter-departmental meetings, in-services, and distribution of program materials

  • Effectively communicate goals, standards, program expectations, service performance & how the work serves Trinity Health objectives

Minimum Qualifications

High school diploma or equivalent

Five (5) years of experience as a Community Health Worker. Paid and non-paid experience/internships are accepted as experience. Education can be considered in lieu of the required work experience.

Must have previous experience with managing people, program, or a project

Must complete CHW Supervisor training within 6 months of employment
