"The SNP Care Management team is responsible for the health care management and coordination of care for SNP members with complex and chronic care needs. The Supervisor, Care Coordination SNP, works with the SNP Manager to assure compliance with CM program requirements as outlined in the Model of Care. This includes the monitoring of activities to support the timeliness of Health Risk Assessment (Initial and Reassessment) completion, Individualized Care Plan (ICP) development and Interdisciplinary Care Team (ICT) delivery for SNP members. Additionally, the Supervisor, Care Coordination SNP, oversees the Care Coordinator SNP team

The Supervisor, Care Coordination SNP, is responsible for the following functions to support the SNP Care Management program as defined below:
"

General Duties/Responsibilities (May include but are not limited to): 

  • Create and maintain an environment that inspires and encourages the growth and engagement of team members.
  • Provide guidance to staff or directly manage complicated coordination requests from members, providers, or staff.
  • Assist with the development and oversight of SNP Care Coordination programs, projects, and workflows.  Development of standard operating procedures (SOP) and reports.
  • Collaborate with other supervisors and managers in the department to improve processes and workflow.
  • Act as a resource to the members, providers, and community partners.
  • Communicate effectively, both verbally and in writing, including presentations as required.
  • Establish and maintain effective interpersonal relationships with staff at all levels, providers, other departments, or programs.
  • Build team engagement
  • Communicate with internal and external customers in a professional manner and follows through on commitments.
  • Initiate and follow through on projects.
  • Work well as part of a team to manage/lead multiple projects simultaneously.
  • Think critically to solve problems effectively.
  • Work closely as a liaison between management and the team to ensure that new cases assigned are touched in a timely manner.
  • Review reports on program performance, identifies reporting opportunities and gaps to ensure the effective delivery of the SNP program. 
  • Participate in SNP quality audits including CMS program audits and vendor audits to assess timeliness of SNP activities. 
  • Reports audit results and identifies opportunities for process improvements
  • Effectively communicate and keep the Care Management leadership team informed of all departmental operations, activities, data, program performance, issues or any other pertinent information that would impact the overall program compliance or achievement of internal goals.
  • Review reporting to assign tasks to Care Coordinators for completion of time sensitive items.
  • Mentor, train, audit, coach and manage a team of non-clinical Care Coordinator staff to ensure compliance with Alignment policies and procedure and all regulatory requirements
  • Direct administrative work as needed, including but not limited to maintaining and updating member’s records, mailing, or faxing correspondence to PCP’s/Specialists, opening cases and tasks in the Care Management platform to meet specific deadlines.  (responds to various workloads by assigning task priorities according to department policies, standards and needs)
  • Assist with Case Management team coverage plans as needed
  • Maintain confidentiality of information between and among health care professionals
  • Other duties as assigned by CM Supervisor, Manager, or Director

SUPERVISORY RESPONSIBILITIES:

Oversees assigned staff. Responsibilities include recruiting, selecting, orienting, and training employees; assigning workload; planning, monitoring, and appraising job results; and coaching, counseling, and disciplining employees.

Minimum Requirements:

Minimum Experience:

3+ years Care Coordination experience; or any combination of education and experience, which would provide an equivalent background.

1 + years supervisory/lead experience in a health care setting or any combination of education and experience, which would provide an equivalent background.

Education/Licensure:

High school diploma or general education degree (GED); with one to three months related experience and/or training; or equivalent combination of education and experience.

No licensure required.

Other:

Ability to analyze data for trends.

Knowledge of Managed Care Plans and medical terminology

Computer Skills, 25 WPM (Microsoft Outlook, excel, word)

Good oral, written and telephone skills

Reasoning Skills:  Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.  Ability to deal with problems involving a few concrete variables in standardized situations.

Computer Skills:  Basic skills required.

Good organizational skills

Ability to reason and carry out instructions.

Good interpersonal skills.

Read, write and speak English fluently.

File systematically.

Pay Range: $58,531.00 - $87,797.00

Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
