POSITION OVERVIEW:

The Patient Care Coordinator serves as a key member of the Spiritual Ministry Department, responsible for leading, coordinating and overseeing and supporting patients staying in the partner patient provided accommodations. The role ensures that patients receive continuous care, appropriate monitoring and coordination between health care provider, the Local Government Unit (LGU) assign and the accommodation in charge. The Patient Care Coordinator act as key point of contact to ensure patient safety, comfort and adherence to treatment plans during their stay outside the hospital.

This position ensures the effective delivery of the patient care integrating Discipleship, Counseling, Education, Life Skills, and Recreation within a safe and nurturing environment. The Coordinator supports participants in experiencing God’s peace (shalom) through holistic transformation and strengthened community relationships.

ESSENTIAL DUTIES & RESPONSIBILITIES:

I. Program Implementation & Delivery

  • Monitor the condition and well being of patients housed in the partner accommodations through their in charge. Act as liason between patients, families, accommodation provider and health care team.
  • Ensure patients accommodation meet required health, safety and hygiene standards.
  • Conduct regular visits in person and follow up call to assess patient status and needs.
  • Identify and escalate any medical concern or psychological concerns to appropriate health care provider.
  • Apply the trauma-informed programs and activities that are age-appropriate, contextualized, and responsive to the needs of children and caregivers, including those with disabilities.
  • Plan and facilitate structured activities such as Bible sessions, trauma-healing groups, life skills lessons, creative arts, and recreation.
  • Ensure proper stewardship and use of ministry and educational resources (e.g., OneHope, trauma-healing tools, learning materials, and craft supplies).
  • Maintain a joyful, safe, and supportive environment that reflects Christian love and dignity.

II. Spiritual and Emotional Restoration

  • Facilitate age-appropriate Bible studies, devotionals, prayer times, and discipleship sessions to guide participants toward a relationship with Christ and biblical understanding of suffering and hope.
  • Provide or coordinate trauma-informed counseling and emotional support to help participants process grief, fear, shame, and rejection.
  • Promote a culture of love, forgiveness, dignity, and mutual respect within accommodation.
  • Integrate faith-based approaches that counter harmful cultural beliefs and affirm God’s healing purpose for every child and family.

III. Participant Assessment and Support

  • Conduct holistic assessments of participants’ physical, emotional, social, and spiritual needs upon entry into the patient accommodation.
  • Provide or facilitate individualized educational and psychosocial support, ensuring children regain learning momentum during their hospital stay.
  • Track progress through entry and exit assessments and maintain accurate participant records to evaluate growth and outcomes.
  • Facilitate smooth transitions for participants, connecting them with local churches and community networks for ongoing discipleship and support after discharge.

IV. Teamwork and Collaboration

  • Collaborate closely with the hospital’s spiritual ministry, counseling, and clinical teams to ensure holistic care for children and caregivers.
  • Engage with community organizations, schools, and churches to enhance program delivery and strengthen referral pathways.
  • Support the orientation and training of volunteers and visiting ministry partners to align them with Patients Accommodation principles and values.
  • Participate in departmental meetings, devotions, and hospital-wide ministry initiatives.

V. Administration and Reporting

  • Maintain accurate program data, attendance records, and ministry reports.
  • Submit monthly and quarterly reports to the Spiritual Ministry Director.
  • Collect participant feedback to inform continuous improvement of Patient Accommodation ministry activities.
  • Assist in documenting transformation stories that demonstrate the impact of patient accommodation.

SECONDARY DUTIES:

  • Uphold the highest standards of quality, efficiency, and child safeguarding in all ministry activities.
  • Adhere to infection prevention and control protocols, ensuring a safe environment for children, caregivers, and staff.
  • Participate in hospital quality improvement and ministry planning processes.
  • Model sustainable and eco-friendly work practices by minimizing waste and responsibly using resources..

OCCUPATIONAL SAFETY & HEALTH:

  • Uphold the highest standards of quality and efficiency in line with hospital quality programs (e.g. Safecare, DOH, etc.) requirements.
  • Ensure a safe and secure work environment by strictly adhering to infection prevention and control protocols such as but not limited to handwashing, wearing of PPE, etc.
  • Identifying, reporting & self-managing hazards where appropriate.
  • Early and accurate reporting of incidents at work and raising issues of concern when identified.
  • Commit to sustainable and eco-friendly work practices by minimizing waste, optimizing resource usage, and actively participating in initiatives that promote environmental stewardship.

EDUCATION/EXPERIENCE REQUIREMENTS:

  1. Diploma or Bachelor’s degree in Theology, Counseling, Social Work or related field (required).
  2. Strong and vibrant Christian faith, aligned with CURE International’s mission to heal the sick and proclaim the Kingdom of God.
  3. At least 2 years of experience in ministry, discipleship, counseling, education, or related work with children and families (preferably those affected by trauma, disability, or chronic illness).
  4. Knowledge and application of trauma-informed principles (safety, trust, collaboration, empowerment).
  5. Understanding of biblical shalom (holistic peace and wholeness) as a foundation for ministry.
 

KNOWLEDGE, SKILLS & ABILITIES: 

  1. A born-again and Mature Christian
  2. Pastoral Care Skills: Having a background in pastoral care, counseling, or a related field to provide spiritual support and guidance to patients and their families.
  3. Empathy and Compassion: Showing genuine care and understanding for the struggles and emotions that patients and families may be experiencing.
  4. Active Listening: Being able to listen attentively and with empathy to the concerns, fears, and needs of patients and families.
  5. Spiritual Guidance: Providing spiritual guidance, prayer, and religious services based on the beliefs and preferences of patients and families.
  6. Cultural Sensitivity: Understanding and respecting the diverse cultural and religious backgrounds of patients, adapting support and services accordingly.
  7. Collaboration with Healthcare Teams: Working closely with healthcare professionals, chaplains, social workers, and other staff to ensure holistic care for patients.
  8. Communication Skills: Effectively communicating with patients, families, and healthcare teams to coordinate care and support services.
  9. Crisis Intervention: Being prepared to offer support and guidance during critical or end-of-life situations.
  10. Resourcefulness: Connecting patients and families with community resources, support groups, and services that can assist them during and after their healthcare journey.
  11. Advocacy: Advocating for patients' spiritual and emotional needs within the healthcare system, ensuring they receive appropriate care and support.
  12. Documentation: Maintaining accurate and confidential records of patient interactions, needs, and services provided.
  13. Training and Education: Keeping up-to-date with best practices in pastoral care and healthcare ethics, attending relevant workshops and training programs.
  14. Volunteer Management: Recruiting, training, and coordinating volunteers who provide additional support and companionship to patients.
  15. End-of-Life Care: Having knowledge and skills in providing spiritual care and support to patients and families facing end-of-life issues.
  16. Collaboration with Religious Leaders: Building relationships with local religious leaders and clergy to support patients of various faiths.
  17. Boundaries and Confidentiality: Understanding and maintaining appropriate boundaries and confidentiality in all patient interactions.
  18. Grief Support: Offering grief support and counseling to patients and families who have experienced loss.
  19. Comforting Presence: Being a comforting and reassuring presence for patients and families during times of uncertainty and distress.
  20. Documentation and Reporting: Keeping detailed records of patient interactions and outcomes for reporting purposes and program evaluation.
  21. Self-Care: Practicing self-care and seeking support to prevent burnout and maintain emotional well-being while providing support to others.
