Position Summary:

The Speech Language Pathologist (SLP) is responsible for providing skilled speech and language therapy services to patients in their homes as prescribed by the attending physician. The SLP evaluates patient conditions, develops individualized treatment plans, and works collaboratively with the interdisciplinary home health team to restore or improve communication and swallowing abilities, enhance patient quality of life, and support safe, effective home care outcomes.

Key Responsibilities:

Clinical Care Delivery

  • Conduct comprehensive assessments of speech, language, cognitive, voice, and swallowing functions.

  • Develop and implement individualized care plans tailored to patient needs and physician orders.

  • Deliver skilled therapy interventions for disorders such as aphasia, dysarthria, apraxia, dysphagia, and cognitive-communication deficits.

  • Educate patients and caregivers on treatment plans, home programs, communication techniques, and compensatory strategies.

Interdisciplinary Collaboration

  • Collaborate with nurses, physical therapists, occupational therapists, physicians, and case managers to coordinate care and support patient goals.

  • Communicate regularly with the clinical team to share updates, revise care plans, and support transitions in care.

Documentation & Compliance

  • Accurately document evaluations, goals, treatment interventions, progress notes, and discharge summaries in the electronic medical record system (Kinnser/WellSky).

  • Ensure all documentation meets regulatory and agency standards and is completed in a timely manner.

Patient & Family Support

  • Provide family and caregiver education to support treatment goals and promote carryover of therapeutic strategies.

  • Offer compassionate, patient-centered care that respects patient autonomy, cultural beliefs, and communication preferences.

Qualifications:

  • Master’s degree in Speech-Language Pathology from an accredited program.

  • Current license as a Speech-Language Pathologist in the state of Texas.

  • Certificate of Clinical Competence (CCC) from ASHA preferred; CFY supervision may be available.

  • Minimum of one year experience in a healthcare setting (home health experience preferred).

  • Current CPR certification.

  • Valid driver's license, auto insurance, and reliable transportation.

Work Environment & Schedule:

  • Home health setting with travel to patient residences.

  • Flexible hours depending on caseload

Physical/Sensory Requirements (with or without the aid of mechanical devices)

Prolonged standing and walking required, with ability to lift up to 50 lbs and move patients. Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition; meet patient and family individualized psychosocial needs. Requires eye-hand coordination and manual dexterity.

Environmental and Working Conditions: 

Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency calls.
