Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. As a 7 On / 7 Off After Hours Registered Nurse, you will:
- Complete an initial assessment of patient and family to determine hospice care needs
- Provide a complete physical assessment and history of current and previous illness
- Use health assessment data to determine nursing diagnosis
- Triage calls and assists patient/primary caregiver with problem resolution; counsel the patient and family in meeting nursing and related needs
- Make visits for symptom management and support as needed
- Modify patient treatment plans as indicated by patients' responses and conditions
- Maintain current, accurate, detailed reports in accordance with policy
- Monitor, record and report symptoms and changes in patients' conditions to the IDG
- Provide health care instructions to the patient as appropriate per assessment and plan
- Obtain and receive physician orders as required
- Pronouncement of patients
- Other duties as assigned
Requirements
- Current license to practice as a registered professional nurse in Georgia
- Current and continued CPR certification
- At least one or two years of recent home care experience; hospice experience highly preferred
- Good verbal and written communication and problem-solving skills
- Responsible valid driver’s license; reliable, insured, automobile for making home visits required
