Essential Responsibilities

• Provide comprehensive primary care services to adult patients, including preventive care, routine examinations, and chronic disease management.

• Evaluate, diagnose, and treat a broad range of acute and chronic medical conditions.

• Develop individualized treatment plans based on patients’ medical histories, clinical findings, and healthcare needs.

• Manage common conditions such as hypertension, diabetes, hyperlipidemia, and other chronic illnesses.

• Order, review, and interpret appropriate laboratory tests, diagnostic studies, and other clinical investigations.

• Emphasize preventive medicine, health screenings, patient education, and lifestyle counseling.

• Coordinate care with specialists and other healthcare professionals to ensure continuity of care.

• Maintain accurate and timely patient records using the electronic medical record (EMR) system.

Qualifications

• MD or DO degree from an accredited medical school.

• Completion of an accredited residency program in Family Medicine or Internal Medicine.

• Board-Certified or Board-Eligible in Family Medicine or Internal Medicine.

• Eligible for Connecticut medical licensure.

• Strong clinical judgment, communication skills, and a commitment to high-quality patient care.

• Ability to work collaboratively with other healthcare professionals in an outpatient clinical setting.
