MDS Coordinator – RN (Per Diem)

Maine Veterans’ Homes is looking for an experienced Registered Nurse (RN) to join our team as a Per Diem MDS Coordinator. This is an excellent opportunity for an RN with long-term care experience who enjoys working independently, managing details, and collaborating with an interdisciplinary team to ensure residents receive the care and services they need.

As an MDS Coordinator, you will play an important role in the Minimum Data Set (MDS) assessment process, ensuring assessments are accurate, complete, timely, and supported by appropriate documentation.

This is an on-site position and is not remote.

What You’ll Do

• Coordinate and complete the MDS assessment process, ensuring accurate, comprehensive, and timely submissions.
• Collect, reconcile, and validate clinical information to ensure MDS data is supported by resident documentation.
• Conduct MDS audits to identify opportunities for improved accuracy, completeness, and compliance.
• Collaborate with nursing leadership and the interdisciplinary team to support quality care and appropriate utilization of resident benefits.
• Maintain required records and documentation in accordance with MVH policies and regulatory requirements.
• Work independently while serving as a knowledgeable resource to the nursing and interdisciplinary teams.

What We’re Looking For

Required:

• Current, unrestricted Registered Nurse (RN) license in the State of Maine
• CPR certification
• RN experience in a long-term care or skilled nursing environment
• Working knowledge of computers, including Microsoft Word and Excel

Experience preferred:

• MDS coordination or MDS assessment experience
• Skilled nursing experience
• Long-term care experience
• Dementia/geriatric nursing experience
• Medicare or regulatory experience

The position description calls for at least five years of progressive professional nursing experience, including at least two years in long-term care, clinical, and charge experience.
