Physical Therapist (PT) – Home Health | Bergen County, NJ | Part-Time & PRN | $65–$85 Per Visit
Stern at Home Therapy is hiring Part-Time and PRN Physical Therapists (PTs) to provide one-on-one care to patients throughout Bergen County, NJ.
This opportunity is ideal for Physical Therapists seeking flexible scheduling, competitive per-visit compensation, and the ability to build a caseload around their availability, without the double-booking or traditional productivity quotas common in other therapy settings.
What You'll Do
As a Physical Therapist with Stern at Home Therapy, you'll evaluate and treat a primarily geriatric patient population in their homes throughout Bergen County.
You'll have full clinical autonomy over evaluations and treatment planning while our dedicated administrative team supports you with referrals, scheduling, authorizations, and patient coordination.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients' homes.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
- Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, families, and members of the interdisciplinary care team.
- Manage your caseload around your availability and preferred Bergen County coverage area.
Compensation:
Part-Time – Minimum 10 Patients
Part-Time W-2:
- $65 per follow-up visit.
- $75 per 4-unit evaluation.
Part-Time 1099:
- $72 per follow-up visit.
- $85 per evaluation.
PRN – Minimum 5 Patients:
- $72 per follow-up visit.
- $85 per evaluation.
Part-Time W-2 Benefits:

Qualifications:
- Active Physical Therapist license in New Jersey required.
- Graduate of an accredited Physical Therapy program.
- Reliable transportation required.
- Strong clinical, communication, documentation, and time-management skills.
- Ability to work independently in a home-based setting.
- Home health or geriatric experience is preferred but not required.
