Overview
The Food Is Medicine (FIM) Registered Dietitian is a new grant-funded position supported through Delaware's Rural Health Transformation Program (RHTP). Serving as the Clinical Nutrition Lead for Southern Delaware's Food Is Medicine (FIM) Program, this position provides leadership for nutrition education services, training of Sussex County’s FIM leadership team of American College of Culinary Medicine (ACCM) practitioners and Community Health Workers (CHWs), FIM pilot implementation planning, and grant outcomes measurement activities. The ideal candidate is a Registered Dietitian with experience in medical nutrition therapy, chronic disease management, nutrition education, community-based health initiatives, and interdisciplinary care coordination, with a demonstrated ability to build partnerships and engage diverse populations. The position is responsible for delivering medical nutrition therapy and nutrition education, supporting development of clinical workflows and referral pathways, coordinating a network of CHWs, contributing to FIM workforce development initiatives, and advancing implementation, evaluation, and long-term sustainability of the Southern Delaware's FIM Program.
This is a grant-funded position. Employment is contingent upon the continued availability and renewal of grant funds.
Responsibilities
Clinical Nutrition Services & Culinary Medicine Education
- Conduct comprehensive nutrition assessments for patients with diet-related chronic diseases and food insecurity.
- Provide individualized medical nutrition therapy (MNT) through in-person counseling sessions, via telehealth, and in FIM teaching kitchens.
- Develop and provide ongoing individualized nutrition care plans based on health conditions, food access needs, cultural preferences, and readiness for change.
- Monitor participant progress and modify care plans as needed.
- Develop and deliver group nutrition education programs and culinary medicine activities, adapting educational materials for diverse populations and literacy levels.
- Teach group culinary medicine hands-on cooking demonstrations, meal preparation education, and healthy eating workshops with a culturally-relevant approach.
- Support implementation of evidence-based culinary medicine programming in collaboration with the American College of Culinary Medicine (ACCM) and Sussex-based clinicians.
- Support nutrition-focused community education special events and outreach activities.
Program Development & Pilot Planning
- Serve as the clinical nutrition subject matter expert for the Southern Delaware FIM Program.
- Lead the design and implementation of standardized nutrition screening, referral, clinical workflows, and care coordination processes.
- Contribute to Food Is Medicine pilot planning, participate in implementation readiness activities, and co-facilitate culinary medicine group medical appointments.
Care Coordination & Community Integration
- Collaborate closely with Community Health Workers to support FIM participant engagement and follow-up.
- Participate in and contribute to interdisciplinary care coordination across FIM partners and ACCM certified clinicians.
- Support development and implementation of referral workflows connecting healthcare clinicians, CHWs, community resources, and Food Is Medicine services.
- Assist participants in accessing available FIM resources, including prescription programs, medically tailored grocery and/or meal programs, and community food resources.
- Collaborate with FIM partner organizations including healthcare systems, federally qualified health centers, community-based organizations, Southern Delaware’s FIM Advisory Council, local farmers and growers, and the Food Bank of Delaware.
Workforce Development & Training
- Pursue and/or maintain Certified Culinary Medicine Specialist (CCMS) certification and training as required by the position.
- Support training and mentoring of Certified Culinary Medicine Specialist (CCMS) healthcare professionals and Certified Culinary Medicine Educator (CCME) Community Health Workers regarding Food Is Medicine interventions.
- Support community partner and clinician engagement activities related to Food Is Medicine implementation.
Data Collection, Evaluation & Reporting
- Document assessments, interventions/ outcomes, care plans for Food Is Medicine participants in EPIC, clinician and patient facing apps, and/or grant-related technology platforms.
- Assist with grant reporting and program evaluation activities.
- Support implementation of performance measurement, quality improvement, and outcome tracking processes.
Qualifications
Minimum Qualifications
- Master's degree in Dietetics.
- Current Registered Dietitian (RD/RDN) credential.
- Licensed RD/RDN in the State of Delaware (or eligible for licensure).
- Certified Culinary Medicine Specialist (CCMS) credential or ability to obtain certification within 12 months.
- Minimum 2 years of clinical, community, public health nutrition, population health, or chronic disease management experience.
- Experience facilitating nutrition education classes, cooking demonstrations, and/or culinary medicine programming.
- Experience documenting services and maintaining clinical records.
Preferred Qualifications
- Experience directly providing medical nutrition counseling and/or in food security, produce prescription, medically tailored meals, or medically tailored grocery initiatives.
- Experience developing workflows, protocols, and clinical programs.
- Experience supporting grants, program evaluation, or population health initiatives.
- Familiarity with Epic and/or electronic medical records systems.
- Experience supervising, mentoring, or training staff.
