Join an exceptional healthcare team and play a vital role in coordinating care, advocating for patients, and ensuring the right care is delivered at the right time.The Case Manager role includes:
- Coordination and management of patient care throughout the entire continuum, from admission through discharge and post-hospital follow-up.
- Collaborate closely with patients, families, physicians, interdisciplinary care teams, and third-party payers to ensure appropriate levels of care, seamless transitions, exceptional patient outcomes, and effective utilization of healthcare resources.
- Serve as a patient advocate while facilitating high-quality, efficient, and compassionate care delivery.
The starting hourly rate for this position is $48.43, which includes a base hourly rate of $39.95 plus an $8.48 geographical differential. In addition to competitive pay, Roswell Park offers a comprehensive benefits package designed to support your health, well-being, and professional growth.

Qualifications:
Required Education and ExperienceLicense Requirement
• License to practice as a Registered Nurse in the State of New York.
• Education and Experience
• 1. Bachelor of Science Degree in Nursing and the equivalent of two (2) years of full time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
• 2. Associate’s degree in Nursing and the equivalent of four (4) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management; or
• 3. Nursing diploma and the equivalent of five (5) years of full-time clinical experience in the field of Utilization Management/Discharge Planning, Home Health Care or Case Management.
