Responsibilities
The RN Case Manager at the direction of the Clinical Manager, screens, evaluates, re-evaluates and treats patients referred by a licensed provider. Coordinates and communicates treatment activities with health care team members and the patient. Works closely with the interdisciplinary team to ensure high-quality patient care.

Qualifications
- Must be a licensed Registered Nurse in Vermont and New Hampshire or hold a compact license.
- Graduate of an accredited school of nursing
- One year of experience as an RN in a home health care setting is preferred
Required Licensure/Certifications
- Must hold at least one active unencumbered RN license in either unencumbered VT or NH at the time of hire. Employees must have active unencumbered RN licenses in both VT and NH within 30 days of their hire date.
- Valid driver’s license with a clean driving record
- Current car insurance, which meets minimum standards
- BLS within 90 days of hire
