SUMMARY OF DUTIES
License holder, authorized by law to dispense drugs and medicines prescribed by consultants or other registered medical personnel. Coordinates and oversees all activities of the services to ensure proper provision and dispensing of medication. Working with other medical personnel to provide treatments and drug information in a manner, which ensures optimal patient care.
MAIN DUTIES RESPONSIBILITIES
1.To understands and carries out pharmaceutical care in line with Vision and mission, Core Values, Goals and objective that suit the scope of the Pharmacy services and the hospital.
2.To understands the service organization structure.
3.To attend meetings held by the services in order to discuss issues and matters pertaining to the operation of the service.
4.To check and sign poison purchase orders prepared by the pharmacy purchasing assistant.
5.To perform, assists and supervises all activities in the pharmacy including dispensing, sales and supply/ dispensing of medications.
6.To contribute to in-service training for healthcare workers and provide counseling on the use and care of prescription drugs to patients.
7.To assess the educational needs of all the staffs and provide necessary training. Gives examination to pharmacy staffs and to test and upgrade their skills. 8.To ensure that Dangerous Drugs and Psychotropic Drugs are handled, stored and issued according to laws and regulations and hospital policies.
9.To issue Psychotropic Drugs to nursing units.
10Responsible in recording and filing of Psychotropic & Dangerous Drugs and to hold the keys of the cupboard.
11.To ensure prescription received are screened and interpreted correctly before dispensing.
12.To perform medication appropriateness review and validate any medication reconciliation when necessary
13.To check and endorse inpatient orders, outpatient prescription and TTA prescription (prescription intervention). T
14 To assist pharmacist in charge in the evaluation of Staff as required.
15.To handle inquiries concerning drug contents, dosages, regimen review errors, storage and disposal.
16.To monitor drug therapies for drug interactions, allergies and contraindications.
17.To monitor and control inventory to ensure that drugs are available when needed and wastage is minimized.

19.To ensure and advises on safe disposal of pharmaceutical products.
20.To participate in quarterly and as when needed stock takes.
21.To check the prepared roster, verifies staff’s annual leave prior to approval from Pharmacist in Charge.
22.To involve in identifying lapses/challenges in the services and contributes in solving and decision making and to conduct Root Cause Analysis in case of Medication Error.
23.To report and document adverse drug reactions, medication errors.
24.Involves in drug inventory procedures, inventory management, purchasing, receiving, drug supplies and medication storage of the services.
25.To keep work area tidy and clean.
26.To be precise and accurate while dispensing and in case of any error occurred to immediately report to Pharmacist in Charge.
27.To uphold best practices by using 7R rules - Right drug, right patient, right frequency, right duration, right route, right information and right documentation.
QUALIFICATIONS REQUIRED
- Bachelor Degree in Pharmacy
- Registered with the Malaysian Board of Pharmacy
- Possess Poison License A
- Able to communicate fluently and write in English and Bahasa Malaysia
EXPERIENCE REQUIRED
- Should demonstrate evidence of internship in hospital setting during his/her undergraduate Degree.
- Working experience in the hospital setting
