GENERAL RESPONSIBILITIES
Provides subject matter expertise and operational support of housing services staff to ensure effectiveness and consistency of program specific procedures, administrative procedures, and adherence to corporate policies.
Coordinates and provides supportive services including but not limited to applications, outreach, home visits, social service referrals, crisis intervention, case management, client advocacy and related services to individuals and families assigned to the specific housing program.
ESSENTIAL DUTIES
- Performs all the duties of the Housing Services job description corresponding to the staff on their team.
- Supports assigned Housing Services personnel to meet the requirements of their respective job descriptions.
- Leads staff meetings and provides consultation regarding individual participant housing services plans.
- Performs participant needs assessments and develops, implements, and monitors service plans for each participant. Assist participants to meet service plan goals and objectives.
- Maintains collaborative linkages with other programs that serve the participants. Linkages will include existing health care providers, local social services agencies, shelter providers, housing services, and other appropriate providers.
- Supports and assists to coordinate Housing Services staff at clinic locations to ensure that policies and procedures are followed.
- Develops training aids and provides on-boarding support for the Housing Services team.
- Assists the Clinical Supervisor or Associate BH Director in the development and implementation of organizational community, outreach, and in-reach initiatives.
- Works closely with Housing Support leadership to monitor team productivity.
- Works with TRHS leaders on project development to increase patient engagement.
- Manages Housing Services housing referrals requiring follow up.
- Assists with the completion and submission of monthly reports required by funders and TRHS.
- Continuously expands knowledge and understanding of community resources and services.
- Works collaboratively with Housing Services staff to establish positive supportive relationship with participants and provides feedback to other members of the team. Builds and maintains positive working relationships with other providers as needed.
- Meets regularly with supervisor and/or Associate BH Director to review activities, identify problems and establish priorities.
MARGINAL DUTIES
- Participates in staff, clinic, corporate meetings, and team meetings as directed by supervisor.
- May participate and provide feedback to leadership for personnel evaluations of assigned case management staff.
- Assists in developing additional services with approval of supervisor.
- Other duties as assigned.

- Is available to provide support to assigned staff in a timely fashion.

- Promotes a positive image of the program to related professionals and the public at all times.
- Contracts and reporting requirements are completed in a timely fashion.
- Treatment policy and procedures are reviewed and updated annually.
- Staff meetings are scheduled and run effectively.
- Confidentiality is maintained and HIPAA policies are always complied with.

Supervised and evaluated by Behavioral Health Leadership
MINIMUM QUALIFICATIONS
The following minimum qualifications are required for this position. All required education, licensure, certifications, registrations, and other job specific credentials must be obtained and successfully verified prior to the employee's start date.
- Three years post-secondary education or three years' experience in lieu of education.
- Experience working with underserved populations; knowledge, commitment, and sensitivity to the needs of people with housing insecurity, substance misuse, mental health challenges or other challenging populations
- A working knowledge of service area social service resources for those patients with behavioral health issues (city, county, state and federal).
- Ability to work independently.
- Excellent communication and customer service skills.
