Social Services Director (Full-Time)

Sunnyvale Gardens Post Acute is seeking a dynamic, compassionate, and experienced Social Services Director to lead our department. Compensation & Benefits

• Salary Range: $75,000 – $100,000 annually (depending on experience)

Key Responsibilities

• Department Leadership: Oversee the day-to-day operations of the Social Services Department, providing guidance and supervision to assistant staff.
• Care Planning & Assessment: Conduct comprehensive psychosocial assessments, develop individualized care plans, and participate in interdisciplinary team meetings.
• Discharge Planning: Manage complex discharge planning processes, ensuring safe, seamless transitions of care back into the community or lower acuity settings.
• Regulatory Compliance: Maintain full compliance with federal, state (CDPH), and local regulations governing long-term care social services.
• Family & Resident Relations: Act as the primary liaison and advocate for residents and families, addressing grievances, rights concerns, and financial resource guidance (Medi-Cal/Medicare).

Qualifications & Requirements

• Education: Bachelor’s degree in Social Work (BSW) or a human services field is required; Master’s degree in Social Work (MSW) is highly preferred.
• Experience: Minimum of 2–3 years of social services experience in a skilled nursing facility (SNF), post-acute rehab, or long-term care setting.
• Leadership Skills: Proven experience managing a department or supervising staff within a healthcare environment.
• Technical Skills: Proficiency with electronic health record (EHR) software (e.g., PointClickCare) and regulatory documentation.
