Location: Springtown / Azle, TX Area.

We're looking for a compassionate Certified Nursing Assistant (CNA) / Hospice Aide to provide PRN support to patients throughout the Springtown and Azle area.

What You'll Do:
- Provide personal care, including bathing, dressing, grooming, and hygiene assistance.
- Assist patients with mobility, positioning, and transfers as directed.
- Help maintain a clean, safe, and comfortable environment for patients
- Observe patients and report changes in condition to the supervising nurse.
- Provide companionship and emotional support to patients and families.
- Follow the patient's individualized plan of care.
- Complete accurate and timely visit documentation.
- Work closely with nurses and the interdisciplinary hospice team to provide compassionate patient-centered care.
What We're Looking For:
- Current Texas CNA certification.
- Hospice, home health, long-term care, or patient care experience preferred.
- Compassionate, dependable, and comfortable providing one-on-one patient care.
- Strong communication and interpersonal skills.
- Reliable transportation, valid driver's license, and current auto insurance.
- Ability to travel throughout the Springtown/Azle service area
- Current CPR certification preferred.
Hospice experience is preferred but not required. If you're a caring CNA with a heart for helping patients and families, we'd love to talk with you.
