Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide Continuous Care to patients in the Broward County area on the following schedule: 7pm - 7:30am and every other weekend. What you'll Do
As a Continuous Care LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care.
- Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
- Coordinate and provide individualized care plans that prioritize comfort and dignity
- Utilize monitoring, intervention, and teaching skills to enhance quality of life
- Provide emotional support and reassurance to patients and families during overnight care
- Ensure exceptional, personalized service and a positive experience for every patient and family

What You'll Need
- Current license to practice as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
