What we want
The Project Director, Value-Based Care, under general direction, directs value-based care project management activities and leads care coordination teams in support of rural hospitals across Texas. Provides consulting and technical assistance to rural healthcare facilities on value-based care, population health, alternative clinical models of care, and alternative payment models. Develops project plans, implements, and tracks project timelines, and deliverables, and provides periodic evaluations and reports for projects to deliver high quality and cost-effective solutions in alignment with the strategic goals and objectives of the Texas A&M Rural and Community Health Institute.

Travel: This position may require up to 50% travel to rural healthcare facilities and other locations.
Schedule: This position may require work beyond normal office hours and/or work on weekends.
Responsibilities:
Value-Based Care Project Management
- Directs value-based care projects and initiatives that support rural hospitals in Texas in transitioning to value-based care, population health, alternative clinical models of care, and alternative payment models. This includes coordinating activities with care coordination teams, rural hospitals, clinics, healthcare consultants, other Texas A&M Health colleges and units, and external stakeholders.
- Approves project plans and ensures projects have clearly defined scopes, budgets, timelines, resource allocation, goals, and deliverables.
- Oversees the allocation of resources (personnel, funding, equipment) across multiple projects, prioritizing initiatives that have the greatest potential for impact in rural communities.
- Establishes performance goals, quality metrics, and measures to evaluate the success of assigned projects, and provides periodic evaluations and reports.
- Ensures quality control and data integrity in all project development and activities, and manages the lifecycle of projects from conception through dissemination of findings.
- Ensures compliance with unit, department, University, System, and government standards, policies, and procedures.
Care Coordination Team Leadership and Supervision
- Directs and supervises care coordination teams supporting rural hospitals, including assigning and reviewing work, evaluating performance, and making recommendations regarding hiring, termination, and other changes in employment status.
- Provides leadership, training, and support to care coordination staff to enhance their capacity to support rural value-based care and population health initiatives.
- Oversees the development of care coordination workflows, training materials, resources, and toolkits for staff, rural healthcare providers, and stakeholders.
- Fosters a collaborative environment and provides guidance and support to team members to achieve project objectives and professional development goals.
Technical Assistance and Outreach
- Provides technical assistance to rural hospitals, clinics, and partners on value-based care, population health, care coordination, alternative clinical models of care, and alternative payment models.
- Works with rural Texas partners, commercial vendors, and public payers to identify viable value-based models of care for rural communities.
- Builds and maintains positive professional relationships and communications with rural health communities, hospital leadership, and county, district, and state staff to promote and facilitate full understanding of program services.
- Coordinates with ACO and CIN partners to monitor participating rural hospitals' performance benchmarks and directs care coordination interventions designed to close identified gaps.
- Develops reports, analyses, and other documents and presents to program leadership and other key stakeholders; represents the department at conferences, committee meetings, and other events.
- Other duties as assigned.
Qualifications:
Required Education and Experience
- Bachelor's degree in healthcare administration, public health, or related field.
- 10 years of experience in healthcare operations, including project management or project coordination experience in a healthcare setting, and experience in value-based care, population health, or care coordination.
Preferred Qualifications
- Master's degree in healthcare administration, public health, or related field.
- Experience working with Clinically Integrated Networks, Accountable Care Organizations, or similar value-based care models.
- Familiarity with quality measurement (HEDIS, CMS Star Rating, MIPS) and risk adjustment (HCC coding)
- Experience partnering with commercial vendors to design and implement programs to improve the wellness of defined populations.
- Experience working with rural or underserved populations.
- Proven experience managing multidisciplinary teams and complex projects in healthcare.
- Knowledge of federal and state regulations in healthcare delivery and funding.
- Background in health IT, including familiarity with electronic health records (EHR/EMR) or telehealth platforms.
- Experience with technical assistance delivery, stakeholder engagement, and capacity-building initiatives.
- Experience in public speaking and presentations.
Knowledge, Skills, and Abilities
- Knowledge and understanding of the rural healthcare environment and the challenges faced by rural hospitals and clinics.
- In-depth knowledge of various alternative payment and alternative care models.
- Working knowledge of regulations and contracts related to commercial and public (Medicare/Medicaid) payers.
- Knowledge of the healthcare environment related to transitions between ambulatory, acute, and post-acute care arenas.
- Ability to use and manage electronic medical records (EMR) systems, and working knowledge of EMR integration systems and quality/performance data.
- Proficient in quality metrics, data management, and reporting.
- Ability to lead and manage teams across multiple programs/projects.
- Ability to build and maintain positive professional relationships with internal and external executive-level clients and stakeholders.
- Strong project management skills, with the ability to multitask and meet deadlines.
- Ability to assess needs, develop plans, and identify appropriate technical assistance resources.
- Strong written and verbal communication skills, with the ability to communicate clearly and effectively to ensure understanding.
- Strong interpersonal, planning, and organizational skills.
- Strong time management, prioritization, problem-solving, and critical thinking skills.
- Proficient in Microsoft Office Suite applications and project management software.
