Description
Newly graduated LVNs are welcome to apply. Candidates with care coordination or case management experience are highly preferred.
Responsibilities:
- Coordinate patient care and services for home health patients.
- Communicate with field clinicians regarding patient needs and care updates.
- Monitor and verify patient authorizations and visit schedules.
- Ensure timely documentation and compliance with agency standards.
- Collaborate with physicians, nurses, and interdisciplinary team members.
- Assist with care planning and coordination of services.
- Active California LVN License (Required).
- Strong communication and organizational skills.
- Ability to multitask and coordinate patient services efficiently.
- Experience in care coordination, case management, or home health is a plus.
- Newly licensed LVNs are encouraged to apply.
