What You'll Do

• Evaluate and develop individualized treatment plans
• Provide evidence-based physical therapy services to adult and geriatric patients
• Help patients regain strength, mobility, balance, and independence
• Collaborate with PTAs, OTs, OTAs, SLPs, nursing, and physicians
• Complete timely documentation in accordance with Medicare and regulatory guidelines
• Participate in interdisciplinary care planning and discharge planning
• Deliver outstanding patient-centered care while achieving positive clinical outcomes

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Michigan Physical Therapist license (or eligible for licensure)
• Strong communication and clinical skills
• Experience in Skilled Nursing, Post-Acute Care, Inpatient Rehab, Home Health, or Outpatient is preferred but not required
• New graduates are encouraged to apply
