Leading Faith-Based, Not-for-Profit Health System | Home Health & Hospice Division

Port Charlotte, FL | Full-Time | Day Shift | On-Site

COMPENSATION RANGE: $96,266 – $179,046 + Comprehensive Benefits Package

If you are leading a home health agency right now, you already know the truth no industry report can fully capture: you are running a high-stakes operation in an environment where caregiver turnover hovers near 80% annually, CMS rule changes arrive faster than your team can absorb them, and every staffing gap quietly threatens your referral pipeline. You are expected to be the clinical authority, the financial steward, the compliance officer, the recruiter, and the hospital liaison — often without the operational backbone to do any of it the way you know it should be done.

This role is built differently. You will lead an established, accredited home health agency that sits inside one of the largest, best-resourced, faith-based health systems in the country — with the back-office infrastructure, talent acquisition support, and clinical reputation that lets a director actually direct, instead of fighting for the basics.

Our client — a Forbes-recognized, not-for-profit health system that serves approximately 4 million unique patients across more than 2,000 care sites and 57 hospitals nationwide — is seeking a Director, Home Health Administrator to lead its Port Charlotte, Florida agency. You will own the day-to-day operations of the agency, oversee clinical, operational, and financial performance, and serve as the primary point of partnership with hospital leaders and care management teams across the West Florida market. You will not be working off the side of a desk in a fragmented agency. You will be plugged into a fully integrated continuum of care with a dedicated talent acquisition partner, a clinical AI advisory infrastructure, and a system-level compliance and quality apparatus that most independent agency administrators can only dream of.

WHAT YOU’LL DO

  • Direct all day-to-day operations of the home health agency, ensuring continuity of care and that a clinical supervisor is available during all operating hours and an RN is available 24/7 for patient concerns.
  • Own the agency’s clinical, operational, and financial performance — including monitoring financial variances, identifying cost and reimbursement opportunities, managing payer credentialing, and leading the agency to meet and exceed national benchmarks on key performance indicators that drive reportable outcomes.
  • Lead and develop a multidisciplinary team — supervising support staff, performing or overseeing the functions of clinical supervisor, intake, scheduling, quality review, utilization management, and medical records as needed.
  • Serve as the agency’s primary partner to hospital leaders and care management teams — strengthening referral relationships, collaborating on patient outcomes, and engineering smoother transitions of care across the system.
  • Maintain full regulatory compliance with state agencies, CMS, CLIA, and accrediting bodies — and lead the QI committee in quarterly reviews of patient safety, infection control, performance improvement, patient experience, and outcome trends.
  • Build and retain a qualified workforce in partnership with system talent acquisition and HR — ensuring all personnel meet state and federal requirements and that retention is treated as a leadership KPI, not an HR afterthought.
  • Develop and implement policies and procedures that genuinely improve operational efficiency — and translate strategic initiatives from the broader health system into agency-level execution.

WHO YOU ARE

Required

  • Bachelor’s degree in Nursing or another healthcare-related field — or 4+ years of equivalent professional experience in lieu of the degree.
  • Active Registered Nurse (RN) license OR active Physical Therapist (PT) license.
  • 5+ years of progressive leadership experience in home-based care or related healthcare operations.
  • Demonstrated clinical expertise, leadership ability, and working knowledge of healthcare community resources.
  • Strong communication and stakeholder-management skills.

Preferred

  • Basic Life Support (BLS) certification.
  • Direct experience leading a Medicare-certified home health agency through PDGM, OASIS, QAPI, and survey readiness.
  • Track record of building hospital and care-management referral relationships in an integrated delivery system.
  • Demonstrated success reducing turnover and stabilizing clinical staffing in a home- or community-based setting.
  • Comfort with EHR optimization, value-based purchasing metrics, and HHCAHPS performance management.
