Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Primary Care Physician for a PACE (Program of All-Inclusive Care for the Elderly) organization in Stockton, California.

POSITION OVERVIEW
• Provide comprehensive primary care for older adults enrolled in a value-based PACE program
• Lead and collaborate with an interdisciplinary care team to develop individualized care plans
• Perform comprehensive in-person assessments and coordinate ongoing medical management
• Coordinate transitions of care including acute care, rehabilitation, and long-term placement
• Conduct scheduled home visits while maintaining continuity of care
• Average approximately eight patients per day, allowing for relationship-focused medicine
• No hospital rounding responsibilities

COMPENSATION
• $316,000-$386,000 Annual Base Salary
• Quarterly Performance Bonus
• Relocation Assistance Available

SCHEDULE
• Monday-Friday
• 8:00 AM-5:00 PM
• Home Visits Included
• Phone Call Rotation Only (A Few Nights/Weekends Per Month)

Requirements

• MD or DO Degree REQUIRED
• Successful Completion of an ACGME Accredited Residency REQUIRED
• Active California Physician License REQUIRED
• Board Eligible or Board Certified in Family Medicine, Internal Medicine, Emergency Medicine, or Geriatrics REQUIRED
• DEA Registration REQUIRED
• California Controlled Substance Registration REQUIRED
• Eligible to Enroll as a Medicare and Medi-Cal Provider REQUIRED
• CPR and First Aid Certification Preferred Upon Hire (Required Within 180 Days)
• Commitment to Caring for Older Adult Populations REQUIRED
