What You'll Do

As a Youth Case Manager, you will:

• Build trusting, supportive relationships with youth and their families while engaging them as active partners in their care.
• Conduct comprehensive assessments and contribute to individualized treatment and care plans.
• Coordinate services with behavioral health providers, medical professionals, schools, social service agencies, and community partners.
• Advocate for youth and families by helping them access resources, benefits, and community supports.
• Serve as a liaison between clients, families, and service providers to ensure coordinated, high-quality care.
• Support youth and families through crisis situations by helping identify solutions and coordinating appropriate interventions.
• Exercise sound professional judgment when evaluating complex situations and determining appropriate courses of action.
• Promote client strengths, resilience, and independence while encouraging positive growth and recovery.
• Develop and maintain strong partnerships with community organizations and referral sources.
• Represent clients effectively when working with courts, educational systems, healthcare providers, and social service agencies.
• Demonstrate cultural awareness, respect, and sensitivity in every interaction.
• Complete timely, accurate documentation that meets organizational, state, and federal requirements.
• Participate actively in supervision, team meetings, professional development, and ongoing training.
• Travel throughout the service area to meet with youth and families in homes, schools, community settings, and other locations as needed.
• Provide direct support and transportation for clients and families when appropriate.
• Stay current on best practices, evidence-based interventions, and applicable regulations, compliance requirements, and ethical standards.
• Perform other duties as assigned to support quality client care.

Requirements

What We're Looking For

Education

• Bachelor's degree in Human Services or a related field required, preferably:
  • Social Work
  • Psychology
  • Rehabilitation Counseling
  • Sociology
  • Counseling
  • Or another closely related discipline

Knowledge of behavioral health theory, evidence-based practices, and emerging treatment approaches is preferred.

Experience

• Previous case management experience in behavioral health, social services, child welfare, or a related field is preferred.
• Experience working with children, adolescents, and families is highly desirable.

• Physical and emotional ability to safely support clients who may exhibit challenging verbal or physical behaviors.
• Proficiency using computers, electronic health records, and other technology required for the position.

Work Environment

This position involves community-based work and regular travel throughout the service area. Some evening, weekend, holiday, or crisis response may be required depending on client needs. The role requires flexibility, sound judgment, and a commitment to providing exceptional care in a variety of settings.
