Summary
Functions as a member of the multi-disciplinary team and coordinates input from the Integrated Care Coordination team and provider practices to provide continuity of care across the care continuum for a defined population. Assesses social determinants of health, identifies needs and barriers, facilitates care plan goal setting, and linkage to resources.

Job Duties

• Performs psychosocial assessments and evaluations to identify and remove barriers to receiving care. Assists patients and families to resources that facilitate care.
• Coordinates referrals between providers to assist patients to meet their goals and improve functioning.
• Identifies outreach opportunities, wellness, and education planning needs of the designated groups.
• Assists to develop coordinated care plans for patients with complex medical and/or behavioral health needs.
• Utilizes shared decision-making models, collaboration, motivational interviewing, and other evidence-based patient engagement interventions to encourage positive behavior change.
• Fosters a team approach by working collaboratively with all practice and community based team members.
• Documents all interventions succinctly and in a timely fashion in the medical record.
• Promotes patient self-management and empowers patients/families to achieve maximum levels of wellness and independence.

Minimum Qualifications

• Master’s Degree Social Work
• 1 year Experience in a medical or clinical setting.
• Ability to recognize the role of cultural, social, economic, and behavioral factors in the accessibility, availability, and delivery of public health services.
• Ability to adjust course of action and plans in relation to others' actions.
• Knowledge of community resources and self help organizations.
• Strong assessment and documentation skills.

Preferred Qualifications

• Clinical training and
• Hospital or ambulatory training
• LSW - Licensed Social Worker - State of Pennsylvania Upon Hire

Physical Demands
Lift and carry 25 lbs. frequent sitting/standing, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
