Job Purpose and Objectives: 

Responsible for providing direct nursing care and public health services to individuals, families and the community for Montgomery County.

Position Requirements:

1. High school diploma or other equivalent

2. Bachelor’s degree in Nursing preferred

3. Three years of nursing experience

4. Two years in grant management or public health program coordination preferred

Certifications / Licenses:

1. Registered Nurse in the State of Kansas

Skills/Knowledge/Abilities:

1. Knowledge of public health and community nursing practices

2. Displays excellent grant reporting and compliance, budgeting and project management skills

3. Proficient in Microsoft Office Suite and public health software

4. Excellent time management skills and ability to multi-task and prioritize work

5. Ability to fulfill all duties with minimal supervision and to work independently

6. Assess situations rapidly and make logical decisions in a timely manner

7. Analyze facts and exercise sound judgment

8. Ability to effectively manage projects and multiple priorities simultaneously

Job Competencies: Essential Functions include the following. 1. Administers immunizations, medications and treatments as prescribed
2. Conducts health assessments, screenings and physical exams
3. Provides nursing care in communicable disease control, maternal/child health, family planning and chronic disease management
4. Collects and manages lab specimens, interpret results and follows up with patients as needed
5. Educates patients and families on health conditions, treatment plans and preventive care
6. Offers counseling and referrals for services such as nutrition, mental health, substance use and family support
7. Participates in community health programs, health fairs and emergency response efforts
8. Provides home visits as part of health promotion activities
9. Maintains accurate and confidential patient records using electronic health systems
10. Documents services in compliance with departmental, state and federal guidelines
11. Reports communicable diseases and other public health concerns as required by law
12. Researches available nursing and public health-related grants at the federal, state and private levels
13. Collaborates with leadership to assess funding needs and prepare grant applications and proposals
14. Develops timelines, budgets and objectives for grant projects
15. Monitors grant expenditures and ensures compliance with all reporting and fiscal requirements
16. Oversees implementation of grant-funded programs and projects
17. Coordinates with internal teams and external partners to achieve grant deliverables
18. Ensures project goals align with the county’s public health priorities and strategic plan
19. Maintains accurate and up-to-date records of grant activity and performance metrics
20. Prepares and submits required reports to funding agencies on time
21. Assists with internal audits and evaluations of grant-funded programs
22. Serves as liaison between nursing/public health staff and grant funding agencies
23. Trains and supports staff on grant-specific procedures and compliance
24. Represents the department at local, regional and state-level meetings as needed
25. Enforces and maintains compliance with all federal, state and local laws and ordinances
26. Performs other duties as assigned or deemed necessary
27. Complies with all organizational and departmental policies and procedures
28. Operates all job-related equipment, machinery, tools and other aids as required or needed
