Experienced Home Health Registered Nurse (RN)

One Home Health is seeking an experienced, compassionate, and independent Home Health Registered Nurse (RN) to join our growing team serving Chicago's North Side, Lincolnwood, Skokie, Morton Grove, Glenview, and Northbrook. 
We are looking for a nurse who is comfortable managing a caseload, providing skilled care in patients' homes, and making sound clinical decisions independently while delivering exceptional patient outcomes.

Job Type: Part-Time, PRN/Per Diem (Opportunities for growth)
Pay: $50.00 - $65.00 per hour (based on experience)
Service Area: North Side Chicago, Lincoln Park, Skokie, Morton Grove, Glenview, and Northbrook, Illinois

Responsibilities

• Manage a caseload of home health patients, performing skilled nursing visits, assessments, and care plan updates.
• Perform admissions, recertifications, resumptions of care, and discharges.
• Complete accurate and timely OASIS assessments and documentation.
• Coordinate care with physicians, therapists, caregivers, and interdisciplinary team members.
• Educate patients and family members on disease management, medications, treatment plans, and home safety.
• Monitor patient conditions and respond appropriately to changes in health status.
• Administer medications, treatments, and skilled nursing interventions as ordered.
• Maintain documentation that meets Medicare, State of Illinois, and agency compliance standards.
• Promote quality patient care and positive health outcomes in the home setting.

Qualifications
Required

• Active, unencumbered Illinois Registered Nurse (RN) license.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Minimum of 1-2 years of RN experience.
• Strong clinical assessment and critical thinking skills.
• Excellent time management, organizational, and communication abilities.
• Ability to work independently and effectively prioritize a full caseload.

Preferred

• Home health experience strongly preferred.
• Knowledge of OASIS documentation and Medicare home health regulations.
• Skilled nursing experience.
• Medical-Surgical (Med-Surg) background.
• Experience with Axxess software.
• Wound care, oncology, or chronic disease management certification.
• Bilingual candidates are encouraged to apply.
