Requirements:
- Active CA Therapy License & 1+ years of licensed experience.
- Valid driver's license, reliable transit, and auto insurance.
- Current CPR certification and professional liability Insurance.
- Recent physical exam (within 2 years) and TB clearance.
- Active EIN and business license/city tax certificate.
