Patient Navigator

Genomic Life is seeking a talented and enthusiastic Patient Navigator to join our team. The Patient Navigator is an individual contributor responsible for facilitating and coordinating personalized navigation services for Genomic Life Members. Working under the Clinical Supervisor, this person will provide initial triage for Genomic Life services, ensure accurate documentation of patient data, and connect patients with appropriate resources to meet their practical, social, physical, emotional, and spiritual needs. The Patient Navigator works in collaboration with Clinical Navigators and the multidisciplinary team, demonstrating active listening skills, empathy, and a strong understanding of evidence-based genomic information.

Location: Remote or Hybrid in San Diego, CA

Salary Range: $50-70k

Responsibilities

• Provide initial triage for Genomic Life services by identifying members’ navigation needs related to genetic testing, cancer diagnosis, or caregiver support
• Act as the primary point of contact for members seeking assistance with accessing benefits, promptly addressing inquiries, and resolving any issues they encounter
• Handle member concerns and complaints with professionalism and empathy, actively listening to their feedback and working towards mutually beneficial resolutions
• Demonstrated ability to effectively prioritize tasks and maintain impeccable organization in a fast-paced environment, ensuring deadlines are met and objectives are achieved efficiently
• Initiate personalized navigation services based on patient and program requirements
• Engage in ongoing professional development to maintain a current working knowledge of evidence-based healthcare and genomic information and support quality-of-care coordination
• Ensure accurate triage documentation and verification of all required patient data using pre-designated tools
• Participate in tracking and monitoring metrics and outcomes, collaborating with administration, and reporting findings to relevant stakeholders
• Communicate patient information appropriately to the Genomic Life Clinical Services team and within the healthcare team
• Identify potential and existing barriers to care and facilitate access to appropriate, credible resources tailored to members’ individual needs, including reading level, health literacy, culture, language, and preferred level of information
• Promote prevention behaviors, early detection, screening, and healthy behavior education among members
• Schedule member appointments for Clinical Navigator and Genetic Counseling as appropriate
• Request and track medical records to facilitate seamless care
• Utilize active listening skills and remain solution-oriented when interacting with patients, families, and members of the healthcare team
• Demonstrate empathy, integrity, honesty, and compassion when engaging in conversations with members
• Demonstrate cultural humility, sensitivity, and responsiveness to diverse patient populations, including gender/gender identity, age, culture, race, ethnicity, religion, abilities, and sexual orientation
• Participate in collaborative team communication and escalate operational challenges to Clinical Navigation leadership to support effective problem-solving and optimize service delivery
• Adhere to all HIPAA guidelines for privacy and confidentiality practices
• Demonstrate knowledge of and adherence to GINA requirements and organizational practices related to the appropriate use, handling, and protection of genetic information

Requirements

• Bachelor's degree in public health, biology, or other related field, OR Associate of Science degree with 5 or more years of healthcare-related experience
• Experience as a Patient Navigator or similar role within healthcare, preferred
• Navigation Training (GW Patient Navigation Training or ACS LION Patient Navigation Training) is preferred
• Knowledge of evidence-based healthcare and patient navigation principles
• Strong communication skills, both verbal and written
• Ability to work collaboratively in a multidisciplinary healthcare team
• Strong interpersonal skills with sensitivity to cultural diversity and the ability to navigate diverse patient populations
• Demonstrates initiative, critical thinking, and is a self-starter
• Detail-oriented, highly organized, able to manage time and competing priorities
• Strong technology skills and quickly learns new technologies; Proficient in Microsoft Office and Zoom, and able to learn new software (Salesforce.com)
