Team Coordinator – Hospice
Vitality Hospice & Palliative Care is seeking an organized, dependable, and compassionate Team Coordinator to support the daily operations of our hospice office. This is a key position that helps keep our clinical team organized, our office running smoothly, and our patients and families supported.
The ideal candidate is a strong multitasker with excellent communication and computer skills who is comfortable working in a fast-paced healthcare environment. Previous experience in hospice, home health, physician office management, healthcare administration, or medical scheduling is strongly preferred.
What You’ll Do
As a Team Coordinator, you will provide administrative and operational support to our clinical and leadership teams. Responsibilities include:
- Coordinate and maintain hospice aide schedules, adjusting schedules as patient census and staffing needs change
- Answer phones and provide professional, compassionate customer service to patients, families, staff, and referral partners
- Assist with daily office functions including scanning, faxing, filing, data entry, and document management
- Maintain accurate and organized patient, clinical, and business records
- Enter and transcribe orders and assist with documentation as appropriate
- Make follow-up calls and assist with coordinating patient needs and transitions of care
- Communicate with nurses, aides, social workers, providers, and other members of the interdisciplinary team
- Assist with referrals and communication between departments
- Support managers and directors with administrative and operational needs
- Assist with technical audits and documentation to help ensure pre-bill readiness
- Maintain office, medical, and printed marketing supply inventory
- Coordinate purchasing and vendor needs while remaining mindful of department budgets
- Process and organize accounts payable documentation and invoices according to company procedures
- Maintain records for company-issued equipment including laptops, phones, keys, and other assigned items
- Assist with regulatory and compliance documentation, including professional licenses and required employee records
- Serve as a liaison between the local office and corporate team
- Participate in staff meetings, team meetings, trainings, and other company activities
- Help create a positive, organized, professional, and welcoming office environment
- Perform additional administrative and operational duties as needed
What We’re Looking For
- Must be at least 18 years of age
- Previous healthcare administrative or office experience preferred
- Experience in hospice, home health, palliative care, physician practices, geriatrics, or another healthcare setting is a plus
- Scheduling or healthcare coordination experience strongly preferred
- Strong computer skills, including Microsoft Word, Excel, and Outlook
- Experience with an Electronic Medical Record (EMR/EHR) system is a plus
- Excellent organization and time-management skills
- Strong written and verbal communication skills
- Professional and compassionate phone etiquette
- Ability to manage multiple priorities and adapt to changing needs
- Detail-oriented with strong documentation skills
- Ability to work independently while also collaborating with an interdisciplinary team
- Dependable, flexible, and able to maintain confidentiality in a healthcare environment
- Must successfully complete required background and employment screenings
