We are seeking a compassionate and dedicated Bereavement Coordinator to join our growing team. In this role, you will provide emotional support, grief counseling, and resources to families and caregivers as they navigate the challenges of loss.

What You'll Do
As our Bereavement Coordinator, you will develop and manage bereavement services to ensure families and loved ones receive the emotional support and resources they need throughout their grief journey.
Key Responsibilities:
- Provide pre-bereavement and grief counseling to patients, families, and caregivers.
- Assess the emotional and bereavement needs of families and develop individualized support plans.
- Provide ongoing bereavement support and follow-up for a minimum of 13 months following a patient's passing.
- Coordinate and facilitate individual counseling, grief support groups, and other bereavement programs.
- Plan and organize memorial services and remembrance events.
- Identify individuals at risk for complicated grief and provide appropriate referrals to community resources and mental health professionals.
- Develop and maintain accurate bereavement records, tracking systems, and documentation.
- Collaborate with nurses, social workers, chaplains, volunteers, and other members of the interdisciplinary hospice team.
- Assist with training and coordinating bereavement volunteers.
- Provide grief education and emotional support to hospice staff when needed.
- Ensure compliance with hospice regulations, company policies, and professional standards.
- Participate in team meetings and occasional evening or weekend on-call coverage as required.
What We're Looking For
We are looking for someone who is compassionate, organized, and comfortable supporting individuals and families during emotionally challenging times.
Qualifications:
- Bachelor's degree in Social Work, Counseling, Theology, or a related field, or specialized certification in bereavement counseling. Master's degree preferred.
- Previous experience providing grief counseling, bereavement support, or working with patients and families experiencing loss.
- Hospice or palliative care experience preferred.
- Strong understanding of grief, loss, and bereavement support.
- Ability to recognize complicated grief and identify when additional mental health support may be necessary.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently while collaborating with an interdisciplinary team.
- Knowledge of community resources and referral services.
- Strong documentation and time-management skills.
- Valid driver's license, reliable transportation, and the ability to travel locally as needed.
Work Schedule & Location
Hybrid
This position offers a combination of remote administrative work and in-person responsibilities, including visits with families, team meetings, grief support groups, and memorial services.
Local travel is required as needed to support patients, families, and hospice programs.
