What You’ll Do
Provide Patient & Family Support
- Assess patients’ psychosocial needs related to illness, environment, and support systems
- Develop and implement individualized care plans focused on emotional, social, and practical needs
- Provide counseling, guidance, and support to patients and caregivers
- Assist patients and families with navigating resources, financial concerns, and care planning
Connect Patients to Resources
- Coordinate referrals to community resources and support services
- Serve as a liaison between patients, families, and community agencies
- Build and maintain relationships with local organizations to support patient needs
Collaborate with the Care Team
- Partner with nurses, physicians, and interdisciplinary team members to ensure holistic care
- Participate in case conferences and contribute to comprehensive care planning
- Support discharge planning to ensure safe and successful transitions
Document & Communicate
- Maintain accurate, timely documentation in the electronic medical record (EMR)
- Communicate patient needs and updates clearly with the care team and referring providers
- Uphold professional standards and regulatory requirements

What You Bring
- Master’s degree in Social Work (MSW) from an accredited program
- Active licensure in the state of practice (as required)
- 1+ year of healthcare experience preferred (home health strongly preferred)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
