Baystate Health is seeking a Med History Tech to join our Pharmacy at Baystate Medical Center in Springfield, Massachusetts.

Required Work Experience:

  •  3- 6 months of experience as a pharmacy technician

 

Required Education:

  • Certified Pharm Tech
  • Highschool diploma or GED

Summary: 

Under the supervision of the care team pharmacist, the pharmacy technician/medication history specialist, interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to Baystate Medical Center. Compiled history information is entered into the patient's electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions that impact the medication reconciliation process and patient care.


Pharmacy technicians, under the supervision of a licensed pharmacist, assist in various activities of the pharmacy department and are responsible for the essential functions and responsibilities outlined in this document, noting every employee is expected to perform other similar and related duties as assigned.  Leadership has the right to change or add to the job responsibilities based on changes in workflows or processes as necessary.


The employee is expected to: Obtain and maintain appropriate knowledge and understanding of Baystate Medical Center/Baystate Health (BH) Pharmacy Services. This knowledge includes the values, policies, and procedures of BMC/BH and BH Pharmacy Services.

Job Responsibilities: 



1) The following are essential job accountabilities:
- Collects an accurate best possible medication list of the patient's home medications in anticipation of hospital admission.
- Gathers information for the best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities) to ensure a complete and accurate entry into the hospital's electronic health record.
- Effectively communicates with other health care professionals to prioritize patient care and identify patient-specific needs.
- Accurately enters the home medication list into the information system for utilization by the provider responsible for admitting the patient.
- Updates the medication list promptly so it can be used as the foundation for the patient's admission medication orders.
- Directs any unresolved issues to the admitting provider.

2) Customer Service, Teamwork, and Communication: Greets, directs, and assists patients, visitors, physicians and ancillary department staff; answers telephones and inquiries by using personable etiquette, or timely refers information to appropriate personnel.


Required Work Experience: 



1) 3- 6 months of experience as a pharmacy technician


Preferred Work Experience: 



1) Experience with medication names, their uses, doses, and routes of administration is strongly preferred


Skills and Competencies: 



1) Possesses excellent communication/interpersonal skills, excellent computer skills and the ability to use various software applications, and excellent patient interview skills


2)  Must be able to work independently and utilizes critical thinking skills and effective time management skills to prioritize tasks

Education:

GED or HiSET (Required)

Compensation

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate's relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.

Minimum - Midpoint - Maximum

$23.46 - $26.97 - $31.91
